摘要
目的探讨小剂量轻比重腰硬联合麻醉对下肢内固定修复术麻醉效果及患者血流动力学的影响。方法选取南方医科大学顺德医院2018年9月至2019年6月收治的60例接受单侧下肢内固定修复手术患者作为研究对象,按照随机数字表法分为观察组和对照组,每组30例。观察组患者采用小剂量轻比重腰硬联合麻醉,对照组采用连续硬膜外麻醉,比较两组患者麻醉效果、血流动力学变化及不良反应发生情况。结果观察组麻醉药物剂量小于对照组,视觉模拟评分法(VAS)评分低于对照组,差异有统计学意义(P<0.05);两组患者麻醉后5min、10min平均动脉压比较,差异有统计学意义(P<0.05);麻醉后30 min、60 min,两组患者平均动脉压比较,差异无统计学意义(P>0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论下肢内固定修复术患者行小剂量轻比重腰硬联合麻醉,可有效减少麻醉药物用量,患者对止血带的耐受性更好,血流动力学稳定,且安全性较高。
Objective To explore the effects of low dose and light specific gravity combined spinal epidural anesthesia on anesthesia and hemodynamics of patients with lower extremity internal fixation.Methods From September 2018 to June 2019,a total of 60 patients in Shunde Hospital of Southern Medical University who underwent unilateral lower extremity internal fixation and repair operation were selected as the study objects.According to the random number table method,they were divided into observation group and control group with 30 cases in each group.The patients in the observation group were treated with low dose and light specific gravity combined spinal epidural anesthesia,while the patients in the control group were treated with continuous epidural anesthesia.The anesthesia effects,hemodynamic changes and adverse reactions of the 2 groups were compared.Results The anesthetic dose in the observation group was smaller than that in the control group,and visual analog scale(VAS)score was lower than the control group,with statistically significant differences(P<0.05).There was no statistically significant difference in the average arterial pressure between the patients in the observation group 5 min,10 min after anesthesia(P<0.05).There was no significant difference in the mean arterial pressure between the two groups(P>0.05),30 minutes and 1 hour after anesthesia and there was no significant difference in the incidence of adverse reactions between the 2 groups(P>0.05).Conclusion Low dose and light specific gravity combined spinal and epidural anesthesia can effectively reduce the dosage of narcotic drugs,and the patients have better tolerance to tourniquet,stable hemodynamics and high safety.
