摘要
[目的]观察云南白药联合去甲肾上腺素、奥美拉唑对重症脑血管病并发应激性溃疡出血的临床疗效。[方法]选取68例重症脑血管病并发应激性溃疡出血患者,随机分为2个组,治疗组(34例)先用8%去甲肾上腺素(8 mg去甲肾上腺素+常温生理盐水100 ml),经胃管注入,每次注入200 ml,1 h后抽出,反复多次直至胃管内抽出淡血性或无色液体,随后注入云南白药0.5 g+生理盐水20 ml,4次/d,同时给予奥美拉唑40 mg,静脉注射,2次/d。对照组给予8%去甲肾上腺素(8 mg去甲肾上腺素+常温生理盐水100 ml),经胃管注入,每次注入200 ml,1 h后抽出,反复多次直至胃管内抽出淡血性或无色液体,同时给予奥美拉唑40 mg,2次/d。[结果]治疗组总有效率为82.35%,对照组为64.71%,组间差异有统计学意义(P<0.05);治疗组止血时间为(24.18±12.37)h,对照组为(57.94±09.82)h,组间差异有统计学意义(P<0.05);治疗组复发率为8%,对照组为23%,组间差异有统计学意义(P<0.01)。[结论]云南白药联合去甲肾上腺素、奥美拉唑治疗重症脑血管病并发应激性溃疡出血疗效显著,无明显不良反应。
[Objective]To observe the clinical efficacy of Yunnan Baiyao combined with norepinephrine and omeprazole on severe cerebrovascular accident and stress ulcer bleeding.[Methods]Sixty-eight cases of severe cerebrovascular accident complicated with stress ulcer bleeding were randomly divided into two groups.The treatment group consisted of 34 cases,first with 8%norepinephrine(8 mg norepinephrine normal temperature normal saline 100 ml),transgastric Tube injection,each time injecting 200 ml,1 hour later,it is taken out repeatedly,until the blood tube is taken out of the blood tube or the colorless liquid,and then injected into Yunnan Baiyao 0.5 g normal saline 20 ml,four times a day,simultaneously given omeprazole 40 mg,intravenous,twice a day.The control group was given 8%norepinephrine(8 mg norepinephrine normal temperature saline 100 ml),injected through the gastric tube,200 ml each time,and extracted 1 hour later,repeated several times until the blood tube was taken out or not.Color liquid,while giving omeprazole 40 mg twice a day.[Results]The total effective rate of the treatment group was 82.35%,and that of the control group was 64.71%,There was significant difference between the groups(P<0.05).The hemostasis time of the treatment group was(24.18±12.37)h,and the hemostasis time of the control group was(57.94±09.82)h,There was significant difference between the two groups(P<0.05),the recurrence rate was 8%in the treatment group and 23%in the control group,and there was a significant difference between the two groups(P<0.01).[Conclusion]Yunnan Baiyao combined and norepinephrine and omeprazole have significant curative effect on severe cerebrovascular accident complicated with stress ulcer bleeding,no obvious adverse reactions.
