摘要
目的探究右美托咪定复合帕瑞昔布在扁桃体等离子消融术的应用效果。方法抽取2017年12月-2021年3月前海人寿广州总医院择期行扁桃体等离子消融术患者60例,根据随机分表法为常规组和观察组,每组30例,常规组全麻后给予右美托咪定0.2μg·kg^(-1)·h^(-1)持续泵注至手术结束,在常规组基础上观察组于麻醉诱导时给予静脉注射帕瑞昔布40mg。对比两组患者拔管前基础状态(T_(0))、拔管时(T_(1))、拔管后30min(T_(2))、60min(T_(3))的平均动脉压(MAP)、心率(HR)、和血氧饱和度(SpO_(2)),观察两组患者T_(2)、T_(3)时Ramsay镇静、疼痛视觉模拟(VAS)评分及躁动率。结果两组患者T_(0)、T_(1)、T_(2)、T_(3)时间点SpO_(2)比较差异无统计学意义(P>0.05),观察组T_(1)、T_(2)、T_(3)时MAP、HR显著低于常规组(P<0.05),观察组T_(2)、T_(3)时Ramsay镇静评分显著高于常规组,VAS评分显著低于常规组,苏醒期躁动发生率明显低于常规组(P<0.05)。结论右美托咪定复合帕瑞昔布应用于扁桃体等离子消融术患者能有效稳定血流动力学,且镇静镇痛效果显著。
Objective To explore the application effect of dexmedetomidine combined with parecoxib in plasma ablation of tonsil.Methods Sixty patients who underwent elective plasma ablation of tonsil in Qianhai Life Insurance Guangzhou General Hospital were enrolled in this study between December 2017 and March 2021.According to random number table method,the patients enrolled were divided into conventional group and observation group with 30 patients in each group.Patients in the conventional group were given continuous infusion of dexmedetomidine at 0.2μg·kg^(-1)·h^(-1)after general anesthesia till the end of operation.On this basis,patients in the observation group were given intravenous injection of 40mg of parecoxib during anesthesia induction.The mean arterial pressure(MAP),heart rate(HR)and blood oxygen saturation(SpO_(2))in basic state before extubation(T_(0)),at extubation(T_(1)),30min(T_(2))and 60min(T_(3))after extubation were compared between the two groups.Ramsay sedation scores and pain visual analogue scale(VAS)scores at T_(2)and T_(3),and restlessness rates of the two groups were observed.Results There was no statistically significant difference in SpO_(2)between the two groups at T_(0),T_(1),T_(2),and T_(3)(P>0.05).The MAP and HR of the observation group at T_(1),T_(2),and T_(3)were significantly lower than those of the conventional group(P<0.05).The observation group had significantly higher Ramsay sedation scores and significantly lower VAS scores than the conventional group at T_(2)and T_(3),and the incidence of restlessness during recovery was significantly lower than that in the conventional group(P<0.05).Conclusion Dexmedetomidine combined with parecoxib can effectively stabilize hemodynamics in patients un dergoing plasma ablation of tonsil,with significant sedative and analgesic effects.
