摘要
目的对糖尿病患者的降糖药物使用情况进行分析,以期为合理用药提供可靠的数据支持并进行指导。方法随机选取2020年1月-2021年1月期间经吉林省桦甸市八道河子镇中心卫生院门诊明确诊断为糖尿病的患者187例并对其处方进行回顾性分析。统计患者的年龄、性别、糖尿病分型以及降糖药物的使用情况,并利用药物利用指数(DUI)对患者降糖药物使用的合理性进行分析。结果参与研究的187例患者中,各年龄段患者所占比例对比差异不明显,其中<60岁的占比51.34%,≥60岁的占比48.66%(P>0.05),男112例,女75例,二者所占比例对比差异显著(P<0.05)。糖尿病类型中以Ⅱ型糖尿病患者居多,占比高达92.51%,研究所涉及到的11种降糖药物中二甲双胍缓释片的使用率最高,达48.66%,用药方案中以两种药物联合治疗的方案占比最多,达到了43.32%,这11种降糖药物的DUI值介于0.33~1.34之间,吡格列酮胶囊的DUI值为1,其余的均在1的上下浮动,其中胰岛素和磺脲类药物的DUI值距离1值最远。结论目前糖尿病患者以Ⅱ型糖尿病者居多,60岁以上患者略多于60岁以下者,患者在治疗过程中多采取两种药物联合治疗的方案,总体来讲能够遵从医嘱以及使用说明用药,但是胰岛素以及磺脲类药物仍存在较为明显的用药不当的情况,需要加强合理用药指导,以期提高患者控糖效果,预防各种并发症发生。
Objective To analyze the use of hypoglycemic drugs in patients with diabetes,in order to provide reliable data support and guidance for rational drug use.Methods 187 patients with diabetes diagnosed in the outpatient clinic of our hospital from January 2020 to January 2021 were randomLy selected and their prescriptions were analyzed retrospectively.The age,sex,classification of diabetes and the use of hypoglycemic drugs were counted,and the rationality of the use of hypoglycemic drugs was analyzed by drug utilization index(DUI).Results Among the 187 patients who participated in the study,there was no significant difference in the proportion of patients of different ages,including 51.34%of patients under 60 years old and 48.66%of patients aged 60 years old(P>0.05).There were 112 male patients,accounting for 59.89%.There were 75 female patients,accounting for 40.11%,and there was significant difference between the two groups(P<0.05).TypeⅡdiabetes is the most common type of diabetes,accounting for 92.51%.Among the 11 hypoglycemic drugs involved in the study,metformin sustained release tablets have the highest utilization rate of 48.66%.The combination of the two drugs accounted for the most,reaching 43.32%.The DUI of these 11 hypoglycemic drugs was between 0.33 and 1.34,and the DUI of pioglitazone capsule was 1.The rest fluctuated up and down at 1,and the DUI values of insulin and sulfonylureas were the farthest from 1.Conclusion At present,most of the patients with diabetes are typeⅡdiabetes,and the patients over 60 years old are slightly more than those under 60 years old.In the course of treatment,the patients often take the combined treatment of two kinds of drugs,and generally speaking,they can follow the doctor’s orders and instructions for the use of drugs.However,insulin and sulfonylureas still have obvious improper drug use,so it is necessary to strengthen rational drug use guidance in order to improve the effect of glucose control in patients,to prevent the occurrence of various complications.
