摘要
目的分析老年卒中后抑郁患者丘脑扩散张量改变特征。方法从2018年1月至2020年6月间,选取我院收治的27例老年卒中后抑郁患者为试验组,另选取27例老年卒中无抑郁患者为对照组。收集患者的临床资料,进行MRI检查和DTI分析,比较两组患者的丘脑体积和FA值,分析丘脑改变和抑郁评分之间的相关性。结果①两组患者的丘脑长度、宽度测量值无明显差异(P>0.05);试验组丘脑体积明显大于对照组,有统计学意义(P<0.05)。②对照组丘脑前核、内侧核、外侧核的患侧FA值均低于健侧,但差异不显著(P>0.05);试验组丘脑内侧核、外侧核的患侧FA值明显低于健侧,有统计学意义(P<0.05)。③Pearson’s关联系数分析显示,丘脑前核的FA值和抑郁评分之间没有显著相关性(P>0.05);丘脑体积、内侧核、外侧核的FA值和抑郁评分之间有显著负相关性(P<0.05)。结论老年卒中后抑郁患者的丘脑亚区体积和扩散张量改变,和抑郁评分具有相关性,可能是引发抑郁的原因。
Objective To analyze characteristics of thalamic diffusion tensor changes in elderly patients with post-stroke depression.Methods The paper chose 27 elderly patients with post-stroke depression in our hospital from January 2018 to June 2020 as experimental group,and 27 cases elderly patients without post-stroke depression as control group.Clinical data of patients were collected and analyzed with MRI and DTI.Thalamic volume and FA value of two groups was compared,and correlation between thalamic changes and depression scores were analyzed.Results①There was no significant difference in thalamus length and width between two groups(P>0.05);thalamus volume of experimental group was significantly larger than control group(P<0.05).②In control group,FA values of affected side of anterior thalamic nucleus,medial thalamic nucleus and lateral thalamic nucleus were lower than healthy side,difference was not significant(P>0.05);in experimental group,FA values of affected side of medial thalamic nucleus and lateral thalamic nucleus were significantly lower than healthy side,with statistical significance(P<0.05).③Pearson’s correlation coefficient analysis showed,there was no significant correlation between FA value of anterior thalamic nucleus and depression scores(P>0.05);there was significant negative correlation between FA value of thalamic volume,medial nucleus and lateral nucleus and depression scores(P<0.05).Conclusion Changes of thalamus volume and diffusion tensor in elderly patients with PSD,and correlation with depression scores may be the cause of depression.
