摘要
目的探讨分析北京市老年人卫生服务现状、卫生服务满意度及其影响因素,为提高北京城乡老年人健康水平和卫生服务水平提供政策建议。方法采取分层、随机、整群、等距的抽样方法对西城区(城区)、大兴区和怀柔区(郊区)≥60岁老年人1837例进行调查。2017年8月,采用综合问卷进行入户调查。结果62.55%的老年人罹患至少2种慢性病。随着年龄的升高,多病共患的发生率亦随之增加(χ~2=36.74,P<0.01)。农村老年人近1年不同就诊频率低于城区,差异有统计学意义(χ~2=157.78,P<0.01)。农村老年人每年自付医疗费用人均6075.61元,城区老年人人均4079.40元,差异有统计学意义(t=-2.45,P=0.01)。老年人对医疗服务的满意度为55.71%。城区老年人就医难的主要原因为等候时间长,占25.48%,农村为经济困难,占39.08%。结论新农合有效地提高了农村老年人的卫生服务可及性,社区卫生服务功能有待于进一步完善。
Objective To understand the current status of community health service for the elderly in Beijing,and to discuss its related influencing factors.Methods A stratified cluster sampling methods was used to randomly select 1837 senior residents aged 60 and above from Xicheng District(urban area),Daxing District and Huairou District(rural areas)in Beijing and an onsite questionnaire survey was conducted in August2018.Comprehensive assessment scale was used to analyze the data.Results 62.55%of the elderly suffered from at least 2 types of chronic diseases.The incidence of concurrent diseases increased with age(χ~2=36.74,P<0.01).The doctor visit rate was lower among the elderly from rural areas compared to those from urban areas in the past 12 months(χ~2=157.78,P<0.01)with significantly lower annual medical expenditure per subject(6075.61 vs 4079.40 RMB,t=-2.45,P=0.01).The overall satisfaction rate for medical service was 55.71%."Waiting too long"was the major difficulty to seek medical service for the elderly from urban areas,accounting for 25.48%,while"not having enough money"was the one for those from rural areas,accounting for 39.08%.Conclusions The new rural cooperative medical insurance system enhances the availability for rural elderly to access health services,while the function of the community health service system needs to be further improved.
