摘要
目的利用"互联网+"构建健康管理模式,实现社区与大型医院共享医疗资源,实现闭环健康管理服务。方法建立"健康小屋",利用"互联网+"手段,构建健康管理信息平台,包括健康档案系统,健康风险评估系统,健康干预系统,健康咨询系统。结果社区开展健康管理是现阶段解决医疗资源共享的有效途径,"互联网+"健康管理模式能实现健康信息的互联互通和健康管理服务的连续性。结论基于"健康小屋"的应用落地,能有效延伸医院医疗服务,提高管理效率,实现健康促进的目标。
Objective Using’Internet+’to build a health management model,enable communities to share medical resources with large hospitals,and realize closed-loop health management services.Methods’Health Hut’was established and’Internet+’method was used to build a health management information platform,including a health file system,a health risk assessment system,a health intervention system,and a health consultation system.Results Community health management was an effective way to resolve medical resource sharing at this stage.The’Internet+’health management model can achieve the interconnection and interconnection of health information and the continuity of health management services.Conclusion The application based on the’Health Hut’can effectively extend hospital medical services,improve management efficiency,and achieve the goal of health promotion.
