摘要
目的探讨不同康复治疗方法对前交叉韧带损伤术后患者短期疗效的影响。方法选择2019年1-8月在新疆医科大学第一附属医院骨科中心行前交叉韧带重建术的30例患者,将患者分为3组,每组10人,采用为常规康复治疗(常规康复治疗组)、常规康复治疗+悬吊运动治疗(常规康复治疗+悬吊运动治疗组)、常规康复治疗+血流控制训练(常规康复治疗+血流控制训练组)3种不同的康复治疗方法,观察并比较3种康复治疗方法对关节镜下前交叉韧带重建术后患者的3个月疗效。结果3组患者均完成康复治疗后,常规康复治疗+血流控制训练组患者健、患侧腿大腿围度差在术后第三个月明显小于常规康复治疗组和常规康复治疗+悬吊运动治疗组,差异有统计学意义(P<0.05)。常规康复治疗+血流控制训练组健、患侧腿大腿围度差在术后第3个月明显小于常规康复治疗组和常规康复治疗+悬吊运动治疗组,差异有统计学意义(P<0.05)常规康复治疗+血流控制训练组LYSHOLM评分和常规康复治疗+悬吊运动治疗组在术后第3个月明显高于常规康复治疗组,差异有统计学意义(P<0.05),常规康复治疗+血流控制训练组健、患侧腿3次连续跳越距离差值和常规康复治疗+悬吊运动治疗组在术后第3个月明显高于常规康复治疗组,差异有统计学意义(P<0.05)常规康复治疗+悬吊运动治疗组健、患侧腿单腿平衡站立时间差在术后第3个月明显高于常规康复治疗组和常规康复治疗+血流控制训练组,差异有统计学意义(P<0.05)常规康复治疗+血流控制训练组健、患侧腿单腿平衡站立时间差在术后第3个月明显高于常规康复治疗组,差异有统计学意义(P<0.05)。结论常规康复治疗+血流控制训练组在大腿围度差、LYSHOLM、健、患侧腿3次连续跳越距离差等方面明显优于常规康复治疗组和常规康复治疗+悬吊运动治疗组,健、患侧腿单腿平衡站立时间差在常规康复治疗+悬吊运动治疗组核心区稳定性测试得分方面明显优于常规康复治疗组和常规康复治疗+血流控制训练组,临床可针对患者个体差异采取不同康复治疗方法。
Objective To explore the effect of different rehabilitation treatment methods on short-term efficacy of patients with anterior cruciate ligament injury.Methods 30 patients were selected who underwent anterior cruciate ligament reconstruction at the orthopedics center of the First Affiliated Hospital of Xinjiang Medical University from January to August 2019.The stratified sampling method divides patients into 3 groups,10 patients in each group,and conventional rehabilitation therapy(conventional rehabilitation therapy group),conventional rehabilitation therapy+suspension exercise therapy(conventional rehabilitation therapy+suspension exercise therapy group),routine rehabilitation treatment+blood flow control training(conventional rehabilitation treatment+blood flow control training group)3 different rehabilitation treatment methods were adopted,the curative effect of three rehabilitation methods on patients with anterior cruciate ligament reconstruction under arthroscope for three months were observed and compared.Results After all three groups of patients completed rehabilitation treatment,the difference between the thigh circumference of the healthy and affected leg in the conventional rehabilitation treatment+blood flow control training group was significantly smaller than that of the conventional rehabilitation treatment group and the conventional rehabilitation treatment+suspension exercise treatment group in the third month after operation.The difference was statistically significant(P<0.05).In the conventional rehabilitation treatment+blood flow control training group,the difference between the thigh circumference of the affected leg and the affected leg was significantly smaller than that of the conventional rehabilitation treatment group and the conventional rehabilitation treatment+suspension exercise treatment group in the third month after operation.The difference was statistically significant(P<0.05)The LYSHOLM score of the conventional rehabilitation treatment+blood flow control training group and the conventional rehabilitation treatment+suspension exercise treatment group were significantly higher than that of the conventional rehabilitation treatment group in the third month after surgery,the difference was statistically significant(P<0.05).The difference between the three consecutive jumping distances of the rehabilitation treatment+blood flow control training group and the affected side leg,and the conventional rehabilitation treatment+suspension exercise treatment group were significantly higher than that of the conventional rehabilitation treatment group in the third month after surgery.The difference was statistically significant(P<0.05).The difference between the standing rehabilitation time of the conventional rehabilitation treatment and the suspension exercise treatment group and the affected leg was significantly higher than that of the conventional rehabilitation treatment group and the conventional rehabilitation treatment+blood flow control training group in the third month after surgery.The difference was statistically significant(P<0.05).The difference between the standing time of the single leg and the balanced leg of the conventional rehabilitation treatment+blood flow control training group was significantly higher than that of the conventional rehabilitation treatment group in the third month after operation.The difference was statistically significant(P<0.05).Conclusion The routine rehabilitation treatment+blood flow control training group was significantly better than that of the routine rehabilitation treatment group and the routine rehabilitation treatment+suspension exercise group in the aspects of poor thigh circumference,Lysholm,health and the distance of three consecutive jumps of the affected side leg.The balance standing time difference of the healthy and the affected side leg in the routine rehabilitation treatment+suspension exercise group was significantly better than that of the routine rehabilitation treatment in the core area stability test score.In the treatment group and the routine rehabilitation+blood flow control training group,different rehabilitation methods can be adopted according to the individual differences of patients.
