摘要
目的评价在唇/颊侧骨板缺损区,行不翻瓣即刻植入种植体后修复的临床效果。方法纳入唇(颊侧骨板缺损患者共7例,共9个手术位点,行不翻瓣即刻种植,植入BICON种植体,种植体植入的同时行骨粉充填骨缺损区,再用胶原膜覆盖。分别于种植当天、6个月行锥束CT检查,分别从近、远中,唇/颊、腭侧四个象限分析种植体周牙槽骨变化。结果9枚即刻种植体在观察期6个月内均无松动脱落,观察期内种植体存留率为100%,并均完成修复。种植体周牙槽骨变化CBCT检测数据:术后6个月唇/颊侧骨板垂直向增加10.17mm^19.01mm,平均增加13.75±1.01mm;唇/颊侧骨板厚度由术前缺损到术后6个月种植体肩台下1mm处为0.75mm^2.88mm,平均骨增量为1.41±0.84mm。术后随访6~12个月,患者行即刻种植后种植体稳定性良好,可正常行使功能。结论在剩余牙槽嵴宽度足够的条件下,临床上对唇/颊侧骨板缺损的病人行微创的即刻种植有较高的成功率,而且可有效增加唇/颊侧牙槽骨量,减少手术次数,缩短疗程,同时还可以降低治疗费用。
Objective To measure the clinical effect of immediate flapless implant into extraction sockets with labial/buccal defects.Methods A total of 9 sockets of 7 patients were included in this study.Each underwent flapless immediate implant placement and reconstruction of the labial/buccal plant with bone graft material in combination with absorbable collagen membrane.Cone beam computed tomography(CBCT)was performed immediately after implant placement(T0),and 6 months following implant surgery(T6)to evaluate the changes of the bone volume.Measurements were taken at four levels:mesial,distal,labial/buccal,and palatal.Results Cumulative survival rate was 100%during the observation period,and all completed the restoration.CBCT measurements of bone changes around the implant:6 months after operation,the labial/buccal bone plate increased10.17 mm^19.01 mm vertically,13.75±1.01 mm on average;The defects of the buccal/lateral bone plate were radiographically reconstructed 0.75 mm^2.88 mm at the 1 mm under the implant shoulder 6-months postoperatively,and the average increasement was 1.41±0.84 mm.The implant had good stability after immediate planting and could perform function normally,after 6~12 month follow-up.Conclusion Within the limitations of this study,flapless implant placement into compromised sockets that the remaining alveolar crest width is sufficient,in combination with GBR grafting may be a viable technique to reconstitute the defected buccal bone plates,and has a preferable success rate.Moreover,this technique may reduce the number of surgeries,shorten the course of treatment,and also reduce the therapeutic cost.This case serious report suggests that,in our clinical practice,we should be very carefully chose the surgical operation for patients with labial/buccal bone plate defects,especially in the anterior aesthetic area.
