超声在评价尿毒症患者血液透析后心脏功能及结构改变的作用

The Effect of Ultrasound in the Evaluation of Cardiac Function and Structure in Patients with Uremia Undergoing Hemodialysis

摘要 目的应用超声对尿毒症患者血液透析前后心脏结构和功能的评价。方法选取2019年6月-2021年9月在我院接受血液透析治疗的62例尿毒症患者,使用超声心动图观察其透析前、透析6月后心脏结构及功能指标的变化。结果与透析前相比,透析6月后左心房、左心室腔室均有缩小,室间隔厚度下降,左心室后壁运动搏幅提升,心包积液减少或消失。与透析前相比,透析6月后,左心房内径、舒张末期左心室内径对比无显著差异(P>0.05);舒张末期左心室后壁厚度和室间隔厚度,左心室心肌重量指数明显下降(P<0.05)。与透析前相比,透析6月后每搏输出量、每分输出量、二尖瓣E峰值、二尖瓣A峰值、E峰和A峰比值无显著差异(P>0.05);左心室射血分数、短轴缩短率明显升高(P<0.05);心肌做功指数明显下降(P<0.05)。结论超声可帮助医师了解早期尿毒症患者心脏损害程度,并可对血液透析治疗后心脏结构和功能的改变进行评价。 Objective:To evaluate the cardiac structure and function in patients with uremia before and after hemodialysis by ultrasound.Methods:A total of 62 patients with uremia who received hemodialysis in our hospital from June 2019 to September 2021 were selected to observe the changes of cardiac structure and functional indexes before and after hemodialysis for 6 months by echocardiography.Results:Compared with before hemodialysis,the left atrium and left ventricular chambers were reduced after 6 months of dialysis,the thickness of interventricular septum decreased,the movement amplitude of left ventricular posterior wall was increased,and pericardial effusion was reduced or disappeared.Compared with before hemodialysis,there was no significant difference in left atrial diameter and left ventricular diameter at the end of diastolic stage after 6 months of hemdialysis(P>0.05).The thickness of left ventricular posterior wall and ventricular septum at the end of diastole stage was significantly decreased(P<0.05).Compared with before hemodialysis,there were no significant differences in stroke output,output per minute,peak value of E,peak value of A,peak value of E and peak value of A after 6 months of dialysis(P>0.05).Left ventricular ejection fraction and shortening rate of short axis were significantly increased(P<0.05).The myocardial work index decreased significantly(P<0.05).Conclusion:Ultrasound can help physicians understand the extent of cardiac damage in early uremia patients and evaluate the changes in cardiac structure and function after hemodialysis treatment.
