摘要
目的研究探讨基于亲情疏导的心理护理联合通乳丹治疗产后缺乳的效果及对患者情绪的改善效果。方法选取2019年6月—2021年6月医院收治的98例产后缺乳产妇为干预对象,分为两组,各49例。对照组产妇给予通乳丹内服+常规护理,观察组在此基础上增加基于亲情疏导的心理护理干预。对两组患者的泌乳量、中医证候评分、负性情绪、护理满意度等进行评估比较。结果观察组治疗第4周、第8周、第12周时的泌乳量均高于对照组(P<0.05)。观察组中医证候积分包括乳汁涩少、神疲乏力、食欲不振、面色无华等症状评分均低于对照组(P<0.05)。干预后观察组患者负性情绪评分包括HAMA-14、HAMD-17评分均低于对照组(P<0.05)。观察组护理满意度高于对照组(P<0.05)。结论基于亲情疏导的心理护理联合通乳丹治疗产后缺乳的效果良好,可增加产妇的产后泌乳量,改善产妇的各项中医证候,提高产妇的心理状态水平,产妇的满意度较高,有较高的护理实践价值。
Objective To study the effect of psychological nursing based on family counseling combined with Tongru Dan in the treatment of postpartum milk deficiency and the improvement of patients’mood.Methods 98 patients were randomly divided into with Hu deficiency from June 2019 to June 2021 observation group and control group,with 49 cases.The control group was treated with Tongru Dan+routine nursing intervention,and the observation group added psychological nursing intervention based on family counseling on the basis of the control group.The lactation volume,TCM symptom score,negative emotion and nursing satisfaction of the two groups were evaluated and compared.Results the lactation volume of the observation group at the 4 th,8 th and 12th weeks of treatment was higher than that of the control group(P<0.05).The scores of TCM symptoms in the observation group,including less astringent milk,mental fatigue,loss of appetite and dull complexion,were lower than those in the control group(P<0.05).The scores of negative emotions including Hamilton anxiety Scale-14(hama-14)and Hamilton Depression scale-17(HAMD-17)in the observation group were(P<0.05).The nursing satisfaction of the observation group was 91.84%(45/49),higher than control group73.47%(36/49)(P<0.05).Conclusion psychological nursing based on family counseling combined with Tongru Dan has a good effect in the treatment of postpartum milk deficiency.It can significantly improve the patient’s lactation,improve the patient’s TCM symptom score,improve the patient’s negative emotion and improve the patient’s nursing satisfaction.It has high clinical application value.
