摘要
目的探究CD19^(+)淋巴细胞以及NK细胞对传染性单核细胞增多症(IM)与慢性活动性EB病毒(EBV)感染的具体价值。方法选取2018-01-01-2021-12-01苏州市相城人民医院收治的EBV感染患者147例,经临床诊断,其中IM84例(IM组),慢性活动性EB病毒感染(CAEBV)63例(CAEBV组),患者均进行CD19^(+)、NK细胞数量检测,并进行EBV病毒载量检测。采用Pearson分析CD19^(+)、CD56^(+)与EBV病毒载量检测的相关性,采用ROC检验,分析CD19^(+)淋巴细胞以及NK细胞对IM、CAEBV鉴别价值。结果IM组CD19^(+)为(11.96±2.35)%、CD56^(+)为(17.05±3.28)%,CAEBV组CD19^(+)为(8.15±1.74)%,CD56^(+)为(12.89±2.41)%,IM组CD19^(+)、CD56^(+)高于CAEBV组,差异有统计学意义,均P<0.05;IM组和CAEBV组患者EBV病毒DNA检测均为阳性,病毒载量分别为(1.57±0.37)×103拷贝/mL、(3.63±0.49)×103拷贝/mL,其中IM组EBV病毒载量低于CAEBV组,差异有统计学意义,P<0.05;经Pearson分析结果显示,CD19^(+)、CD56^(+)与EBV病毒载量均呈负相关,r值分别为-0.424和-0.441,均P<0.05;经ROC曲线分析,CD19^(+)、CD56^(+)对于鉴别IM、CAEBV的AUC值分别为0.655、0683,95%CI为0.677~0.835、0.654~0.854,二者联合的AUC值为0.748,95%CI为0.718~0.904。结论CD19^(+)淋巴细胞及NK细胞存在相关性对于IM、CAEBV的鉴别均有一定价值,其中联合检测用于IM、CAEBV鉴别的价值更高。
Objective To explore the value of CD19^(+)lymphocytes and NK cells in the infection of Infectious mononucleosis(IM)and chronic active Burkitt’s Epstein-Barr virus(CAEBV).Methods A total of 147 patients with EBV infection admitted to Suzhou Xiangcheng People’s Hospital from January 1,2018 to December 1,2021 were selected.Eighty-four patients were diagnosed with IM and 63 with CAEBV.CD19^(+)and NK cell number and EBV viral load were detected in all patients.Pearson analysis was used to analyze the correlation between CD19^(+),CD56^(+)and EBV viral load detection,and ROC test was used to analyze the differential value of CD19^(+)lymphocytes and NK cells on IM and CAEBV.Results CD19^(+)and CD56^(+)were(11.96±2.35)%and(17.05±3.28)%respectively in IM group,and they were(8.15±1.74)%and(12.89±2.41)%respectively in CAEBV group.CD19^(+)and CD56^(+)in IM group were higher than those in CAEBV group(P<0.05).The EBV viral DNA test of patients in the IM group and the CAEBV group were positive,and the viral load of the IM group was lower than that of the CAEBV group(P<0.05).The viral load was(1.57±0.37)×10~3 copies/mL in the IM group and(3.63±0.49)×10~3 copies/mL in the CAEBV group.Pearson analysis showed that CD19^(+),CD56^(+)and EBV viral load were negatively correlated(r=-0.424,r=-0.441,P<0.05).ROC analysis showed that the AUC values of CD19^(+)and CD56^(+)for the identification of IM and CAEBV were 0.655,0683,95%CI were 0.677-0.835,0.654-0.854,the AUC values of the combination of CD19^(+)and CD56^(+)were 0.748,and 95%CI were 0.718-0.904.Conclusion The correlation between CD19^(+)lymphocytes and NK cells is of certain value in the identification of IM and CAEBV,and the combined detection is of higher value in the identification of IM and CAEBV.
