摘要
目的脑卒中幸存患者中70%~80%留有不同程度的残疾,严重影响生活质量。本研究分析脑梗死患者社区康复运动应用效果。方法选取2017-01-01-2018-12-31深圳市蛇口人民医院出院的脑梗死患者180例,按康复治疗方案不同分为观察组与对照组,每组90例。观察组以弹力带、Bobath球、平衡垫、瑜伽垫等器材,制定标准化运动方案,组织患者进行以运动功能为主、兼顾平衡与柔韧性的社区康复运动;对照组仅根据指导与随访自行锻炼。两组分别于康复前及康复3个月后,进行肌力耐力、关节活动度、肌张力、偏瘫运动功能、步行能力、平衡功能和日常生活活动能力(activity of daily living,ADL)评估,以评价两组患者干预效果。结果干预3个月后,观察组肌力得分为(4.51±0.60)分,高于对照组的(3.63±0.64)分,t=9.431,P<0.001;关节活动度得分为(3.60±0.49)分,高于对照组的(2.93±0.44)分,t=9.530,P<0.001;肌张力得分为(0.34±0.56)分,低于对照组的(1.37±0.79)分,t=-10.029,P<0.001;偏瘫运动功能得分为(5.27±0.76)分,高于对照组的(3.63±0.64)分,t=15.539,P<0.001;步行能力得分为(4.04±1.22)分,高于对照组的(2.21±1.13)分,t=10.444,P<0.001;平衡功能得分为(4.27±1.07)分,高于对照组的(3.02±1.57)分,t=6.211,P<0.001;ADL得分为(85.44±19.90)分,高于对照组的(57.28±25.32)分,t=8.298,P<0.001。结论社区康复运动可有效恢复脑梗死患者运动能力和生活能力。
OBJECTIVE There are 70%-80%of stroke survivors have different degrees of disability,which seriously affects the quality of life.This study analyzes the effect of community rehabilitation exercise in patients with cerebral infarction.METHODS A total of 180 patients with cerebral infarction who were discharged from Shenzhen Shekou People’s Hospital from January 1,2017 to December 31,2018 were selected and divided into observation group and control group according to different rehabilitation treatment schemes,90 cases in each group.The observation group used elastic bands,Bobath balls,balance mats,yoga mats and other equipment to formulate standardized exercise prescriptions and organize patients to carry out rehabilitation exercises that focus on motor function and balance both balance and flexibility.The control group exercised only according to guidance and follow-up.Before and after 3 months of rehabilitation,the muscle endurance,joint mobility,muscle tone,hemiplegic motor function,walking ability,balance function,and activity of daily living(ADL)evaluation to evaluate the intervention effect of the two groups of patients.RESULTS After 3 months of intervention,the muscle strength of the observation group reached(4.51±0.60),which was higher than that of the control group(3.63±0.64),t=9.431,P<0.001.The joint activity score was(3.60±0.49),which was higher than that of he control group(2.93±0.44),t=9.530,P<0.001.The muscle tone score was(0.34±0.56),which was lower than that of the control group(1.37±0.79),t=-10.029,P<0.001.The hemiplegic motor function score was(5.27±0.76),which was higher than that of the control group(3.63±0.64),t=15.539,P<0.001.The walking ability score was(4.04±1.22)points,which was higher than that of the control group(2.21±1.13),t=10.444,P<0.001.The balance function score was(4.27±1.07),which was higher than that of the control group(3.02±1.57),t=6.211,P<0.001.The ADL score was(85.44±19.90),which was higher than that of the control group(57.28±25.32),t=8.298,P<0.001.CONCLUSION Community rehabilitation exercise could effectively restore motor function and life ability of patients with cerebral infarction.
