血浆同型半胱氨酸水平与冠心病及冠脉病变发生相关性

Correlation between plasma homocysteine and severity of coronary heart disease

摘要 目的既往研究结果显示,血浆同型半胱氨酸水平是老年高血压患者冠心病发生的独立危险因素,本研究主要分析冠心病发生的因素以及血浆同型半胱氨酸水平与冠脉病变的相关性。方法选取2017-06-01-2018-12-01福建医科大学附属漳州市医院就诊的174例心绞痛患者作为研究对象,其中131例确诊为冠心病患者作为病例组,另43例非冠心病患者作为对照组。检测两组受试者血浆同型半胱氨酸水平,并根据冠脉造影结果判定冠脉病变程度,采用Spearman相关性分析判定血浆同型半胱氨酸水平与冠脉病变程度的相关性,并采用多因素回归分析判定冠心病的独立危险因素。结果血浆中同型半胱氨酸水平从低到高依次为冠脉正常者(9.8±3.5)μmol/L,单纯冠脉粥样硬化者(12.9±4.2)μmol/L,单支病变组(16.6±3.8)μmol/L,双支病变组(20.5±4.4)μmol/L,三支病变组(23.1±5.6)μmol/L,完全堵塞组(25.8±4.3)μmol/L,差异有统计学意义,F=6.835,P=0.0035;Spearman相关分析结果显示,血浆同型半胱氨酸水平与Gensini评分呈正相关(r=0.534,P<0.05);多因素分析结果显示,吸烟史(OR=3.438,95%CI为1.910~6.190,P=0.009)、三酰甘油(OR=1.684,95%CI为1.283~2.210,P=0.003)、高密度脂蛋白(OR=1.883,95%CI为1.322~2.683,P=0.012)、低密度脂蛋白(OR=1.510,95%CI为1.215~1.876,P=0.015),收缩压(OR=2.010,95%CI为1.647~2.452,P=0.007)和同型半胱氨酸水平(OR=2.065,95%CI为1.719~2.479,P=0.001)是冠心病发生的独立危险因素。结论冠心病患者中血浆同型半胱氨酸水平越高,冠状动脉病变程度越严重。血浆同型半胱氨酸水平是冠心病发生的独立危险因素。 OBJECTIVE Previous studies have shown that plasma homocysteine level is an independent risk factor for coronary heart disease in elderly patients with hypertension.This study analyzed the factors of coronary heart disease and the correlation between plasma homocysteine levels and coronary artery disease.METHODS A total of 174 patients with angina pectoris admitted to Zhangzhou Affiliated Hospital of Fujian Medical University from June 1,2017 to December 1,2018 were selected as research objects.In this study,131 coronary heart disease patients were included in the observation group,43 patients without coronary heart disease were treated as the control group.Plasma homocysteine levels were measured in both groups.The correlation between homocysteine level and the severity of coronary artery disease was determined by pearman correlation analysis.RESULTS Plasma homocysteine levels from low to high were normal coronary artery(9.8±3.5)μmol/L,simple coronary atherosclerosis(12.9±4.2)μmol/L,single-vessel disease group(16.6±3.8)μmol/L,double-vessel disease group(20.5±4.4)μmol/L,three-vessel disease group(23.1±5.6)μmol/L,complete occlusion group(25.8±4.3)μmol/L,the difference was statistically significant,F=6.835,P=0.0035.Spearman correlation analysis showed that plasma homocysteine level was positively correlated with Gensini score(r=0.534,P<0.05).Multivariate analysis showed smoking history(OR=3.438,95%CI:1.910-6.190,P=0.009),triacylglycerol(OR=1.684,95%CI:1.283-2.210,P=0.003),high density lipoprotein(OR=1.883,95%CI:1.322-2.683,P=0.012),low-density lipoprotein(OR=1.510,95%CI:1.215-1.876,P=0.015),systolic blood pressure(OR=2.010,95%CI:1.647-2.452,P=0.007)and homocysteine The level of amino acid(OR=2.065,95%CI:1.719-2.479,P=0.001)were independent risk factors for coronary heart disease.CONCLUSIONS The higher plasma homocysteine level,the more severe coronary artery disease.Plasma homocysteine is an independent risk factor for coronary heart disease.
