微创接骨板接骨术治疗肱骨干骨折临床观察 被引量:1

Clinical observation of minimally invasive bone plate osteotomy for humeral shaft fracture

摘要 目的肱骨干骨折对患者上肢功能影响较大,本研究探讨微创接骨板接骨术治疗肱骨干骨折的疗效,为肱骨干骨折治疗提供依据。方法选取2017-01-01—2018-01-01安阳市第二人民医院收治的64例肱骨干骨折患者为研究对象。将患者按照性别和年龄等因素分为对照组与研究组各32例,研究组采用微创接骨板接骨术治疗,对照组采用切开复位内固定的方式进行干预,以上肢功能评分(disability of the arm,shoulder and hand questionnaire,DASH)和手术效果来评估患者的临床治疗效果。结果治疗前研究组DASH评分差异无统计学意义,P>0.05;治疗后,研究组DASH评分为(16.58±1.85)分,低于对照组的(23.14±4.84)分,差异有统计学意义,t=14.198,P<0.001;研究组手术时间为(72.45±4.19)min,低于对照组的(82.58±5.28)min,差异有统计学意义,t=8.501,P<0.001;研究组的X射线暴露时间为(4.66±1.33)s,低于对照组的(38.28±4.91)s,差异有统计学意义,t=37.387,P<0.001;研究中的骨折愈合时间为(12.11±1.33)周,低于对照组的(16.31±1.43)周,差异有统计学意义,t=12.166,P<0.001。结论微创接骨板接骨术治疗肱骨干骨折,手术时间较传统手术短,且手术治疗效果佳,骨折愈合时间显著缩短。 OBJECTIVE The humeral shaft fracture has a great influence on the upper limb function of the patient.This study is to investigate the effect of minimally invasive bone plate osteosynthesis in the treatment of humeral shaft fracture and provide a basis for the treatment of humeral shaft fracture.METHODS Sixty-four patients with humeral shaft fractures admitted to the Second People′s Hospital of Anyang City were selected as subjects from January 1,2017 to January 1,2018.The patients were divided into control group and study group according to factors such as gender and age.The study group was treated with minimally invasive bone plate osteosynthesis,and the control group was treated with open reduction and internal fixation.Clinical treatment effect of patients was evaluated by disability of the arm,shoulder and hand questionnaire(DASH)and surgical effect.RESULTS The DASH score was not statistically significant between pre-treatment group and control group(P>0.05).After treatment,the DASH score of the study group was16.58±1.85,lower than that of the control group 23.14±4.84,the difference was statistically significant(t=14.198,P<0.001).The operation time of the study group was(72.45±4.19)min,which was lower than that of the control group(82.58±5.28)min,and the difference was statistically significant(t=8.501,P<0.001).The X-ray exposure time of the study group was(4.66±1.33)s,lower than that of the control group(38.28±4.91)s,and the difference was statistically significant(t=37.387,P<0.001).The fracture healing time in the study was(12.11±1.33)weeks,which was lower than that in the control group(16.31±1.43)weeks,and the difference was statistically significant(t=12.166,P<0.001).CONCLUSIONS Minimally invasive bone plate osteosynthesis for the treatment of humeral shaft fractures is shorter than traditional surgery,and the surgical treatment is better.The fracture healing time of patients is significantly shortened.
