摘要
目的通过观察晚期胰腺癌患者生存情况,评价健脾散结方对晚期胰腺癌生存期的影响。方法采用非随机同期对照临床研究,以是否接受健脾散结方治疗分为中药组和非中药组,并进一步以有无接受化疗进行分层分析。在基线均衡的条件下比较接受健脾散结方联合化疗的中药组和仅接受化疗的同期非中药组的生存期。单因素以及多因素回归模型分析影响晚期胰腺癌预后的独立因素,Kaplan-Meier法和Log-rank检验估算中位总生存时间(Overall survival time,OS)。结果共纳入晚期胰腺癌患者165例,其中中药组109例,非中药组56例,全组中91例患者出现终点事件,占55.2%。健脾散结方、化疗、Karnofsky(KPS)功能状态评分、性别为影响晚期胰腺癌预后的独立性因素。以是否行姑息化疗分层:化疗亚组中,中药组的中位OS为16.0个月,较非中药组的10.7个月延长,差异有统计学意义(P=0.005,Log-rank检验)。非化疗亚组中,中药组的中位OS为9.7个月,较非中药组的4.1个月延长,差异有统计学意义(P=0.007,Log-rank检验)。结论健脾散结方中药是影响晚期胰腺癌预后的独立性保护因素,单独应用或联合化疗均可改善晚期胰腺癌患者的生存期。
Objective To observe the survival of patients with advanced pancreatic cancer and evaluate the effect of JianpiSanjie Decoction on the survival of advanced pancreatic cancer.Methods By adopting non-randomized simultaneous case-control method,Patients with advanced pancreatic adenocarcinoma were divided into traditional Chinese medicine(TCM)group and non-TCM group according to whether they received JianpiSanjie Decoction or not;and further stratified analysis was carried out according to whether they received chemotherapy or not.The survival time of JianpiSanjie Decoction combined with chemotherapy and chemotherapy only group were compared under the condition of baseline equilibrium.Univariate and multivariate regression models were used to analyze the independent prognostic factors of advanced pancreatic cancer.Kaplan-Meier and Log-rank test were used to estimate the median overall survival time(OS).Results Among the 165 patients with advanced pancreatic cancer,109 were TCM group and 56 were non-TCM group.A total of 91 patients(55.2%)died.JianpiSanjie Decoction,chemotherapy,KPS score,Gender were independent prognostic factors affecting the survival of patients with advanced pancreatic adenocarcinoma.Stratified analysis was performed with or without chemotherapy,In the chemotherapy group,the median OS of the TCM group was 16.0 months,which was significantly longer than that of the non-TCM group by 10.7 months(P=0.005,Log-rank test).In the non chemotherapy group,the median OS of TCM group was 9.7 months,which was significantly longer than that of the non-TCM group by 4.1 months(P=0.007,Log-rank test).Conclusion JianpiSanjie Decoction was independent protective factors to affect survival prognosis.Meanwhile,JianpiSanjie Decoction alone or combined with chemotherapy can improve the survival time of advanced pancreatic cancer.
