摘要
目的探讨电针治疗慢传输型便秘大鼠(Slow transit constipation,STC)的可能机制。方法将40只雄性SD大鼠按随机数字表法分为空白组(10只)、STC组(30只)。采用复方地芬诺酯灌胃法复制STC模型,造模14天后评价造模效果,造模成功后STC组大鼠随机分为模型组(10只)、电针组(10只)、假电针组(10只)。电针组采用毫针交替针刺同侧"大肠俞"、"天枢",后接韩式电针仪(HANS-200E)。假电针组针刺腧穴同电针组,夹持电极但不予通电。以上两组每次均干预30 min,每日1次,5次为1个疗程,疗程之间休息2天,共治疗10次。空白组、模型组仅抓取固定。干预结束后观测各组大鼠首粒黑便排出时间及小肠推进率,Western Blot检测结肠Cajal间质细胞(Interstitial cell of Cajal,ICC)标志物C-kit蛋白及自噬相关蛋白Beclin1、P62的表达水平。结果与空白组比较,模型组大鼠首粒黑便排出时间延长(P<0.01),小肠推进率明显降低(P<0.01),结肠组织C-kit蛋白、P62蛋白表达下调,Beclin1蛋白表达上调(P<0.01);与模型组比较,电针组大鼠首粒黑便排出时间缩短(P<0.01),小肠推进率明显升高(P<0.01),结肠组织C-kit蛋白、P62蛋白表达上调,Beclin1蛋白表达下调(P<0.01);与假电针组比较,电针组大鼠首粒黑便排出时间减少(P<0.05),结肠组织C-kit蛋白、P62蛋白表达上调,Beclin1蛋白表达下调(P<0.01)。结论电针可能通过抑制结肠ICC过度自噬,促进ICC数量和结构的恢复,从而改善STC模型大鼠肠道动力障碍。
Objective To explore the possible mechanism of electroacupuncture treatment for slow transit constipation(STC)in rats.Methods 40 SD rats were randomly divided into the blank group and STC group.The STC model was replicated by gavage with compound diphenoxylate,and the modeling effect was evaluated 14 days after modeling.After successful modeling,the rats in the STC group were randomly divided into model group(n=10),electroacupuncture group(n=10)and sham electroacupuncture group(n=10).In the electroacupuncture group,the acupuncture points on the ipsilateral side of the Dachangshu(BL25)and Tianshu(ST25)were alternately acupunctured with milli needles,followed by the Korean electroacupuncture instrument(HANS-200 E).In the sham electro-acupuncture group,the acupuncture points were the same as those in the electro-acupuncture group,and the electrodes were held but not energized.Both groups intervened for 30 min each time,once a day,5 times for a course of treatment,with a 2-day break between courses,for a total of 10 treatments.The blank group and model group were fixed by grasping only.The expression levels of the interstitial cell of Cajal(ICC)marker C-kit protein and autophagy-related proteins Beclin1 and P62 were detected by Western Blot.Results Compared with the blank group,the first defecation time of the model group increased,the small intestine propulsion rate decreased,the expression of C-kit protein and P62 protein decreased,the Beclin1 protein increased(P<0.01).Compared with the model group,the first defecation time of the electroacupuncture group decreased,the small intestine propulsion rate increased,the expression of C-kit protein and P62 protein increased,the Beclin1 protein decreased(P<0.01).Compared with the sham electroacupuncture group,the first defecation time of the electroacupuncture group decreased(P<0.05),the expression of C-kit protein and P62 protein increased,the Beclin1 protein decreased(P<0.01).Conclusion Electroacupuncture could promote colonic propulsion function to improve symptoms of constipation.The mechanism may be to restore the number of cells by inhibiting excessive autophagy of Cajal interstitial cells.
