摘要
目的:分析健脾止泻汤治疗对小儿迁延性腹泻临床疗效、症状积分及免疫功能的影响。方法:选取选取2017年8月~2019年8月于本院接受诊治的98例迁延性腹泻患儿临床资料,按照治疗方法的差异分为研究组(n=50,健脾止泻汤)与对照组(n=48,蒙脱石散治疗)。比较两组临床疗效、症状积分(大便性状、大便次数、腹痛、发热、呕吐)、免疫功能[免疫球蛋白A(Immunoglobin A,IgA)、免疫球蛋白G(ImmunoglobinG,IgG)、及T淋巴细胞亚群:CD_(4)^(+)/CD_(8)^(+)]、症状改善时间(腹痛缓解时间、呕吐缓解时间、退热时间、正常排便时间及治疗疗程)及不良反应(脱水、恶心呕吐、便秘)。结果:①研究组临床总有效率(96.00%)高于对照组临床总有效率(75.00%)(P<0.05)。②治疗后,2组大便性状、大便次数、腹痛、发热、呕吐等各症状积分均显著改善,且研究组改善优于对照组(P<0.05);③治疗后各组IgA、IgG和CD_(4)^(+)/CD_(8)^(+)水平较治疗前显著升高,研究组IgA、IgG、CD_(4)^(+)/CD_(8)^(+)水平均高于对照组(P<0.05);④研究组患儿腹痛、呕吐、退热、排便及治疗疗程等症状缓解时间上明显优于对照组,差异具有统计学意义(P<0.05);⑤两组治疗期间的不良反应均比较轻微,两组不良反应发生率比较无差异(P>0.05)。结论:对迁延性腹泻患儿予以健脾止泻汤治疗可显著改善患儿症状积分,提升免疫功能,临床疗效佳,值得临床推广。
Objective:To analyze the effect of Jianpi Zhixie Decoction on clinical efficacy,symptom score and immune function for children with metastatic diarrhea.Methods:The clinical data of 98 children with metastatic diarrhea who were diagnosed and treated in our hospital from August 2017 to August 2019 were selected and divided into a study group(n=50,Jianpi Zhixie Decoction)and a controlled group(n=48,Montmorillonite Powder treatment).The clinical efficacy,symptom scores(fecal traits,stool frequency,abdominal pain,fever,vomiting),immune function[Immunoglobin A(IgA),Immunoglobin G(IgG),and T lymphocyte subsets:CD_(4)^(+)/CD_(8)^(+)],improvement time of symptoms(time of abdominal pain remission,time of vomiting remission,antipyretic time,normal bowel time and treatment course)and adverse reactions(dehydration,nausea and vomiting,constipation)were compared between the two groups.Results:(1)The total clinical effective rate of the study group(96.00%)was higher than that of the controlled group(75.00%)(P<0.05).(2)After treatment,the scores of stool traits,stool frequency,abdominal pain,fever,and vomiting in the two groups significantly improved,and the improvement of the study group was better than that of the controlled group(P<0.05).(3)After treatment,the levels of IgA,IgG,and CD_(4)^(+)/CD_(8)^(+)in each group were significantly higher than those before treatment.The levels of IgA,IgG,and CD_(4)^(+)/CD_(8)^(+)in the study group were higher than those in the controlled group(P<0.05).The remission time of symptoms abdominal pain,vomiting,abatement of fever,defecation and treatment course of children in the study group was significantly better than that of the controlled group,and the difference was statistically significant(P<0.05).(5)The adverse reactions during the treatment in the two groups were relatively mild,and there was no difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Using Jianpi Zhixie Decoction in the treatment of children with metastatic diarrhea can significantly improve the symptom score of the children and enhance the immune function.The clinical effect is good,and it is worthy of clinical promotion.
