摘要
目的探讨肺炎支原体肺炎患儿继发支气管扩张的临床特点及影响因素。方法选取2020年6月至2023年6月河北省儿童医院住院的172例重症支原体肺炎患儿为研究对象,以急性肺炎支原体后3个月CT检查结果是否发生支气管扩张将患儿分为为扩张组(n=72)和对照组(n=100)。分析2组患儿年龄、热程、喘息、胸腔积液、肺部病变部位及类型、肺外并发症发生情况,采用多因素Logistic回归分析探究肺炎支原体肺炎患儿继发支气管扩张的影响因素。结果扩张组热程≥10 d、胸腔积液、中下肺实变比例及C反应蛋白高于对照组,发病7 d内使用大环内酯类抗生素、发病7 d内使用支气管镜比例低于对照组(P<0.05)。热程(OR=1.560)、胸腔积液(OR=1.261)、肺部实变部位(OR=1.082)、C反应蛋白(OR=1.042)是重症肺炎支原体肺炎患儿继发支气管扩张的影响因素(P<0.05)。结论热程≥10 d、胸腔积液、C反应蛋白升高、中下肺叶实变为重症支原体肺炎患儿继发支气管扩张后遗症的危险因素,对于具有上述危险因素的患儿应尽早行支气管镜检查及治疗,减轻继发支气管扩张等后遗症的发生。
Objective:To investigate the clinical characteristics and influencing factors of secondary bronchiectasis in children with mycoplasma pneumoniae pneumonia.Methods:A total of 172 children with severe mycoplasma pneumonia hospitalized in Hebei Children's Hospital from June 2020 to June 2023 were selected as the study objects.The children were divided into dilated group(n=72)and control group(n=100)according to the results of CT examination 3 months after acute mycoplasma pneumonia.Age,heat course,wheezing,pleural effusion,location and type of lung lesions,and occurrence of extrapulmonary complications were analyzed in the 2 groups.Multivariate Logistic regression analysis was used to explore the influencing factors of secondary bronchiectasis in children with mycoplasma pneumonia.Results:The heat duration≥10 d,pleural effusion,middle and lower lung compactness ratio and C-reactive protein in dilated group were higher than those in control group,and the proportion of macrolide antibiotics used within 7 days of onset and bronchoscopy used within 7 days of onset were lower than those in control group(P<0.05).Heat duration(OR=1.560),pleural effusion(OR=1.261),lung solid site(OR=1.082),C-reactive protein(OR=1.042),was an influential factor in stimulating bronchiectasis in children with severe mycoplasma pneumoniae pneumonia(P<0.05).Conclusion:Heat duration≥10 days,pleural effusion,elevated C-reactive protein,and middle and lower lung lobular thickness are risk factors for secondary bronchiectasis sequelae in children with severe mycoplasma pneumonia.Bronchoscopy and treatment should be performed as soon as possible for children with these risk factors to reduce the occurrence of secondary bronchiectasis sequelae.
