摘要
目的:探讨归因训练联合多元化肺部康复训练在心脏瓣膜置换术(HVR)术后康复中的应用价值。方法:选择我院2021年7月至2023年7月收治的190例行HVR的患者,按随机数字表法分为对照组(n=95)与观察组(n=95),对照组术后采用常规肺部康复训练,观察组术后采用归因训练联合多元化肺部康复训练,两组均干预至出院后1个月。对比两组归因方式、自我管理能力、心肺功能及运动耐力。结果:干预后,观察组的归因方式问卷(ASQ)正性事件评分高于对照组,负性事件评分低于对照组,差异均有统计学意义(P<0.05);观察组自我管理量表各维度评分均高于对照组,差异均有统计学意义(P<0.05);观察组无氧阈时摄氧量(VO_(2)at)、氧脉搏(O_(2)pulse)、峰值摄氧量(VO_(2)Peak)均高于对照组,差异均有统计学意义(P<0.05);观察组Fugl-Meyer运动量表(FMA)评分高于对照组,6min步行试验(6MWT)长于对照组,差异均有统计学意义(P<0.05)。结论:归因训练联合多元化肺部康复训练在HVR患者术后康复中应用价值显著,可明显改善患者归因方式,提高自我管理能力,促进心肺功能及运动耐力的恢复。
Objective:To explore the application value of attribution training combined with diversified pulmonary rehabilitation training on postoperative rehabilitation of heart valve replacement(HVR).Methods:190 patients undergoing HVR in our hospital from July 2021 to July 2023 were selected and classified into control group(n=95)and observation group(n=95)by means of the random number table method.The control group was treated with routine pulmonary rehabilitation training after surgery,while the observation group was given attribution training combined with diversified pulmonary rehabilitation training after surgery.Both groups were intervened until 1 month after discharge.The attribution style,self-management ability,cardiopulmonary function and exercise tolerance were compared between both groups.Results:After intervention,the positive event score of Attribution Style Questionnaire(ASQ)in observation group was higher than that in control group while the negative event score was lower than that in control group(P<0.05).The scores of dimensions of self-management scale were higher in observation group compared to control group(P<0.05).The observation group had higher oxygen uptake at anaerobic threshold(VO_(2)at),oxygen pulse(O_(2)pulse)and peak oxygen uptake(VO_(2)Peak)(P<0.05).The Fugl-Meyer Assessment(FMA)score in observation group was higher,and the 6-minute walking test(6MWT)was longer than that in control group(P<0.05).Conclusion:Attribution training combined with diversified pulmonary rehabilitation training has significant application value on postoperative rehabilitation of patients with HVR.It can significantly improve the attribution style,enhance the self-management ability,and promote the recovery of cardiopulmonary function and exercise tolerance.
