摘要
目的分析湖北省血吸虫病传播风险,为制定下阶段防治策略提供依据。方法选择2018-2020年湖北省1600个血吸虫病风险监测村开展风险监测,分析监测村的螺情、野粪及联防联控区血吸虫病传播风险情况,并对风险监测结果进行Ⅰ~Ⅲ等级风险评级。结果2018-2020年,全省在46个县,378个乡镇,2091个村,20641处环境开展了风险监测。累计查螺3187560框,捕获螺数605949只,其中活螺数285484只,活螺平均密度为0.0896只/0.11 m^(2)。连续三年活螺平均密度变化不大,各年均未查到阳性钉螺。2018-2020年,采集的野粪总数量呈逐年下降趋势。2019年发现三处阳性野粪环境(牛粪),阳性野粪1份,其他野粪均为阴性。大部分联防联控区家畜放牧和贩卖情况调查不足,传染源管控存在一定薄弱环节。开展风险监测的20641处环境中,2018-2020年查出1处Ⅰ级风险(2019年),285处Ⅱ级风险,其他风险评级均为Ⅲ级风险(最低风险)。结论目前湖北省血吸虫病传播风险处于较低水平,但流行区钉螺孳生环境未彻底改变,需加强风险监测。
Objective To analyz the transmission risk of schistosomiasis in Hubei province,and provide the basis for formlating the control strategy in the next stage.Method A total of 1600 schistosomiasis risk monitoring villages in Hubei province were selected for risk monitoring from 2018 to 2020,and analyze the snail situation,wild feces and schistosomiasis risk,and risk monitoring results were rated fromⅠ-Ⅲ.Results From 2018 to 2020,the province had carried out risk monitoring in 46 counties,378 towns,2091 villages and 20641 environmental sites;the number of checked snails were 3187560 boxes,and 605949 snails were captured with 285484 live snails.The mean density of live snails was 0.0896/0.11 m^(2).The mean density of live snails changed little for three surveillance years.Schistosome infected snails were not found.The total quantity of wild manure collected decreased year by year.In 2019,one positive field manure environment was found(cow dung),and one positive field manure was found,while the others were all negative.Most of the joint prevention and control areas lacked of livestock grazing and trafficking investigations,and there were some weak links in the control of infectous sources.Risk monitoring was conducted 20641 environmental sites;one was found to had a Grade I risk(2019),285 GradeⅡrisk while the rest were rated GradeⅢ(minimum risk)from 2018 to 2020.Conclusion At present,the transmission risk of schistosomiasis in Hubei provinec was low,but the environment of snails in the endemic areas had not changed completely;risk monitoring should be strengthened.
