摘要
目的分析1975-2016年美国癌症患者自杀率的时间趋势,为预防或干预癌症患者的自杀倾向提供借鉴参考。方法基于美国国家癌症研究所建立的监测、流行病学和最终结果(SEER)数据库,提取1975—2016年4715965例癌症患者相关数据,采用JoinPoint计算标准化自杀率、年度百分比变化率(APC)和年平均百分比变化率(AAPC),分析癌症患者自杀的时间趋势。结果1975-2016年,癌症患者的标准化自杀率呈下降趋势,AAPC为-4.6%,P<0.001;就性别而言,男性和女性癌症患者标准化自杀率的时间趋势差异有统计学意义,χ^(2)=52.78,P<0.001,男性癌症患者标准化自杀率的年平均下降幅度高于女性(男性:AAPC为-4.7%,女性:AAPC为-4.4%,均P<0.001),但男性癌症患者的标准化自杀率始终高于女性;年龄方面,各个年龄组癌症患者标准化自杀率的时间趋势差异有统计学意义,χ^(2)=459.68,P<0.001;≥75岁癌症患者的标准化自杀率高于其他年龄组,且该组癌症患者的标准化自杀率下降幅度最小,AAPC为-1.3%,P<0.001。结论1975-2016年,癌症患者的标准化自杀率虽逐年降低,但仍是自杀干预的重点人群,尤其是男性患者和≥75岁的老年患者,要加强预防和干预。
Objective To analyze the time trend of the standardized suicide rate among cancer patients in the United States from 1975 to 2016,and to provide reference for preventing or intervening the suicidal tendency of cancer patients.Methods Based on the SEER database established by the National Cancer Institute of the United States,the relevant data of cancer patients from 1975 to 2016 were collected,and the standardized suicide rate,annual percentage change rate(APC)and annual average percentage change rate(AAPC)were calculated by JoinPoint software to analyze the temporal trend of cancer patients’suicide.Results The standardized suicide rate of cancer patients showed a decreasing trend from 1975 to 2016(AAPC=-4.6%,P<0.001).By gender,there was a significant difference in the time trend of standardized suicide rate between male and female cancer patients(χ^(2)=52.78,P<0.001),the standardized suicide rate of male cancer patients decreased more than that of females(male:AAPC=-4.7%,P<0.001;female:AAPC=-4.4%,P<0.001),but the standardized suicide rate of male cancer patients was always higher than that of females;By age,there were significant differences in the time trend of standardized suicide rate among cancer patients in different age groups(χ^(2)=459.68,P<0.001),the standardized suicide rate of cancer patients aged 75 and above was higher than that of other age groups,and the standardized suicide rate of cancer patients in this group had the smallest decrease trend(AAPC=-1.3%,P<0.001).Conclusion From 1975 to 2016,although the standardized suicide rate of cancer patients decreased year by year,cancer patients are still the key population for suicide intervention,especially for male and elderly cancer patients;therefore,prevention and intervention should be strengthened.
