摘要
目的探讨肩锁关节脱位患者带袢钢板与锁骨钩钢板治疗的效果及对肩峰撞击征发生率的影响比较。方法选取2017年1月—2019年12月本院收治的肩锁关节脱位患者40例,应用随机数字表法将其分为两组,每组20例。对照组应用锁骨钩钢板治疗,研究组应用带袢钢板治疗。比较优良率、并发症、手术结果、肩峰撞击征发生率、肩关节活动度以及肩功能评分。结果两组患者的优良率比较差异无统计学意义(P>0.05),研究组并发症的发生率低于对照组(P<0.05);研究组手术时间长于对照组(P<0.05),研究组切口长度、恢复工作时间以及术中出血量等指标均明显优于对照组(P<0.05),同时对照组患者出现3例肩峰撞击征,研究组出现1例肩峰撞击征,研究组肩峰撞击征发生率低于对照组(P<0.05);术前两组患者的肩关节活动度比较差异无统计学意义(P>0.05),术后研究组肩关节活动度显著优于对照组(P<0.05);术前两组患者的肩功能评分比较差异无统计学意义(P>0.05),术后研究组肩功能评分显著高于对照组(P<0.05)。结论带袢钢板与锁骨钩钢板治疗肩锁关节脱位患者在改善肩关节优良率疗效相当,但是带袢钢板可以显著提高肩关节活动度,改善患者肩功能。
Objective To investigate the effect of looped plate and clavicular hook plate on acromioclavicular impingement in patients with acromioclavicular joint dislocation.Methods 40 patients with acromioclavicular dislocation admitted to the hospital from January 2017 to December 2019 were selected and divided into two groups using random number table method.The control group was treated with clavicular hook plate,and the research group was treated with looped plate.The good and good rates,complications,surgical results,incidence of acromion impaction,shoulder range of motion and shoulder function scores were compared.Results There was no significant difference in the excellent and good rates between the two groups(P>0.05),and the incidence of complications in the study group was lower than that in the control group(P<0.05).Team operation time is longer than the control group(P<0.05),the team of incision length,recovery time and intraoperative blood loss and other indicators are significantly better than those of control group(P<0.05),while the patients peak hit,3 cases of shoulder,and 1 case of shoulder peak hit team,the team shoulder peak impact tax incidence were lower than the control group(P<0.05);There was no significant difference in the range of motion of the shoulder joint between the two groups before surgery(P>0.05),and the range of motion of the shoulder joint in the study group was significantly better than that in the control group after surgery(P<0.05).There was no significant difference in the preoperative shoulder function score between the two groups(P>0.05),and the postoperative shoulder function score in the study group was significantly higher than that in the control group(P<0.05).Conclusion The efficacy of looped plate and clavicular hook plate in the treatment of acromioclavicular joint dislocation in the improvement of the excellent rate of shoulder joint,but looped plate can significantly improve the degree of motion of shoulder joint,improve the shoulder function of patients.
