腰椎迭代重建低剂量CT图像质量和辐射剂量分析与评价

Analysis and evaluation of image quality and radiation dose of lumbar low dose CT with iterative reconstruction

摘要 【目的】评价低剂量扫描、图像空间迭代重建(iterative reconstruction in image space,IRIS)腰椎CT的图像质量及辐射剂量。【方法】回顾性搜集2018年7月到2019年2月期间114例低剂量(130 m As,130 k Vp)扫描、IRIS重建和112例常规剂量(190m As,130 k Vp)扫描、滤过反投影(filtered back projection,FBP)重建、临床怀疑腰椎间盘病变的患者,比较2组患者的信噪比、对比噪声比、主观图像噪声、图像锐利度、诊断可接受性、观察者间的一致性及其辐射剂量。【结果】低剂量组腰3椎体、腰3~4椎间盘、硬膜囊及腰大肌的信噪比显著高于常规组(P<0.001),腰3~4椎间盘、硬膜囊及腰大肌的对比噪声比显著高于常规组(P<0.001),测量者间的一致性较好或很好,组内相关系数(intra-class correlation coefficient,ICC)值为0.739~0.979。低剂量组主观图像噪声、腰3~4椎间盘及椎旁软组织边缘锐利度均优于常规组(P<0.001),诊断可用性评价均为完全能够满足诊断需要,观察者间的一致性很好(kappa值0.806~0.929),低剂量组容积CT剂量指数、剂量长度乘积及有效辐射剂量显著低于常规剂量组(P<0.001)。【结论】采用IRIS迭代重建能够在辐射剂量减少约30%的情况下获得满足诊断需要的腰椎间盘CT图像质量。 【Objective】To evaluate the image quality,radiation dose of lumbar low-dose CT(LDCT)with iterative reconstruction in image space(IRIS).【Methods】Retrospectively collected 114 patients who underwent LDCT(130 m As,130 k Vp)with IRIS and 112 patitents who underwent standard dose CT(SDCT,190 m As,130 k Vp)with FBP for suspected lumbar interval disc lesion from July 2018 to February 2019.The signal-to-noise ratio(SNR),contrast-to-noise ratio(CNR),subjective image noise,image sharpness,diagnostic acceptability and radiation dose between the two groups were compared.Interobserver agreement was also calculated.【Results】The SNR of the 3 rd lumbar vertebra,intervertebral disc between 3 rd and 4 th vertebrae,dural sac and psoas major in the low dose group were higher than those in the conventional group(P<0.001).The CNR of the intervertebral disc between 3 rd and 4 th vertebrae,dural sac and psoas major in the low dose group were higher than those in the conventional group(P<0.001).The consistency between the two measurers was better or very good(ICC 0.739-0.979).While the subjective image noise,marginal sharpness of intervertebral disc between 3 rd and 4 th vertebrae and paravertebral soft tissue of the low dose group were better than those of the conventional group(P<0.001).The diagnostic availability in both groups fully met the diagnostic demands.The consistency between the two observers were perfect(kappa value 0.806-0.929).The volume CT dose index,dose-length product and effective radiation dose in low dose group were significantly lower than those in conventional group(P<0.001).【Conclusion】IRIS iterative reconstruction can achieve diagnostic lumbar disc CT image quality with a reduction in radiation dose of approximately 30%.
