经尿道输尿管镜激光碎石和气压弹道碎石治疗输尿管结石的疗效和安全性比较的Meta分析 被引量:2

摘要 目的采用Meta分析比较经尿道输尿管镜激光碎石术(transurethral ureteroscopic laser lithotripsy,TULL)和经尿道输尿管镜气压弹道碎石术(transurethral ureteroscopic pneumatic lithotripsy,TUPL)治疗输尿管结石的疗效和安全性。方法搜索中国知网、万方、维普、PubMed、Embase、Medline、Elsevier、Wiley Online及Cochrane Library数据库,收集、浏览题目及摘要初筛、精读全文再筛、提取及评估2018年5月30日至2023年5月30日的中文文献和2020年5月30日至2023年5月30日的英文文献,使用RevMan 5.2软件进行统计分析。结果纳入28篇文献,共4214例输尿管结石患者,得出TULL与TUPL在手术时间(P=0.0005)、住院时间(P<0.00001)、血尿素氮(blood urea nitrogen,BUN)(P<0.0001)、血清肌酐(serum creatinine,Scr)(P<0.0001)、术中出血量(P<0.00001)、C反应蛋白(C-reactive protein,CRP)(P=0.02)、结石排净率(P<0.0001)上,差异有统计学意义,且TULL均优于TUPL,具有更少的术后并发症。结论在经尿道输尿管镜治疗输尿管结石中,TULL比TUPL碎石更高效、更安全。 Objective Meta-analysis was performed to compare the efficacy and safety of transurethral ureteroscopic laser lithotripsy(TULL)and transurethral ureteroscopic pneumatic lithotripsy(TUPL)for the treatment of ureteral calculi.Methods By searching China National Knowledge Infrastructure,WanFang Database,VIP Database,PubMed,Embase,Medline,Elsevier,Wiley Online,Cochrane Library,collecting and browsing the preliminary screening of titles and abstracts,and re-screen the full text closely,the Chinese literature from May 30,2018 to May 30,2023 and the English literature from May 30,2020to May 30,2023 were extracted and evaluated.RevMan 5.2 was used for statistical analysis.Results28 articles were included,with a total of 4214 patients.The operative time(P=0.0005),hospital stays(P<0.00001),blood urea nitrogen(BUN)(P<0.0001),serum creatinine(Scr)(P<0.0001),bleeding in the operation(P<0.00001),C-reactive protein(CRP)(P=0.02)and stone removal rate(P<0.0001)were statistically different between TULL and TUPL.TULL was better than TUPL,with fewer postoperative complications.Conclusion TULL is more effective and safer than TUPL in the treatment of ureteral calculi by transurethral ureteroscopy.
