摘要
五苓散出自《伤寒论》,原方主要用来治疗下焦蓄水证或水逆证,核心病机为三焦膀胱气化不利。其临床表现为小便不利、渴欲饮水、水入即吐等症。"肾为气之根""肾为水脏""肾主水"则表明全身的水液代谢离不开肾的固摄作用,五苓散能够健脾化湿、温阳化气、补土制水,以助肾之固摄之力。慢性肾脏病多有水肿、乏力、尿血及尿浊等症状,在现代药理研究中,五苓散的成分能减轻其临床症状及生化指标。在肾病综合征、慢性肾炎、慢性肾衰竭、糖尿病肾病和慢性肾衰竭透析后都有广泛的应用。现对五苓散在慢性肾脏病中的研究进行综述,以期能够更好的开发并充分利用其功效,并促进经典方药的进一步研究及扩大其应用范围,使其不仅能够在肾病领域发挥独特的优势,而且也能使用"异病同治"理念在其他疾病中发挥积极的治疗作用。
Wuling Powder(五苓散)is from Treatise on Febrile Diseases.The original prescription is mainly used to treat lower-Jiao water storage syndrome or water resistance syndrome.The core pathogenesis is adverse to Sanjiao and bladder Qi transformation.Its clinical manifestations are adverse urination,thirst for drinking water,water into the vomit and so on.“Kidney being the root of Qi”“the kidney is the water organ”and“the kidney governing water”indicate that the hydration metabolism of the whole body is inseparable from the firmness of the kidney.Wuling Powder can strengthen the spleen and dissolve dampness,warm the Yang and change the Qi,and cultivating earth to control water to help the firmness of the kidney.Chronic kidney disease has edema,fatigue,urine blood and urine turbidity and other symptoms,in the modern pharmacological research,Wuling Powder components can reduce its clinical symptoms and biochemical indicators.It is widely used in nephrotic syndrome,chronic nephritis,chronic renal failure,diabetic nephropathy and chronic renal failure after dialysis.This review aims to better develop and make full use of its efficacy,promote the further study of classic formulas and expand its application scope,so that it can not only give play to its unique advantages in the field of kidney disease,but also play an active role in the treatment of other diseases by using the concept of“the same treatment for the different disease”.
