摘要
目的探讨山豆根中毒性脑病的临床表现、影像学表现、诊断、治疗及预后。方法收集就诊于河北医科大学第二医院神经内科的1例山豆根中毒性脑病的临床资料,分析这种疾病的症状、体征、影像学表现、诊断、治疗原则、临床转归等。结果山豆根中毒性脑病是一种较为少见的中毒性脑病,临床上可表现为共济失调、眩晕、意识障碍等,影像学主要以双侧基底核、小脑齿状核对称性受损为特征性表现,尚无统一的诊断标准及治疗方案,治疗原则上急性期可给予0.9%氯化钠注射液洗胃等治疗促进毒物代谢,继之以营养神经、补液等对症支持治疗,轻症患者如本例患者经过及时诊断治疗可能获得良好的预后。结论山豆根中毒性脑病患者应根据其过量服用山豆根病史以及共济失调等特征性临床表现和双侧基底核、小脑齿状核对称性受损等影像学表现尽早诊断,及时给予促进毒物代谢以及营养神经等对症治疗,有利于改善患者预后。
Objective To investigate the clinical manifestation,radiological feature,diagnosis,treatment and prognosis of vietnamese sophora root toxication encephalopathy.Methods One case of vietnamese sophora root toxication encephalopathy was reported by our team in Department of Neurology,Second Hospital of Hebei Medical University,and related literatures were summarised to review this disease.Results Vietnamese sophora root toxication encephalopathy is one of rare toxication encephalopathies in our country with not fully understood pathogenesis,which usuallly manifest ataxia and dizziness.Bilateral lesions in basal ganglia and dental nucleus is characteristic neuroimage finding of this disease.Timely diagnosis and treatment such as gastric lavage and emetic method can improve the prognosis.Conclusion Neurologists should be more vigilant on this kind of encephalopathy in order to improve the prognosis of patients.
