摘要
目的基于中医传承辅助平台(v2.5)软件,分析孙增涛教授辨证治疗肺系疾病的用药规律。方法收集2017年9月1日—2018年12月31日孙增涛教授门诊病例,包括主诉、现病史、辅助检查、舌脉和处方,将其录入中医传承辅助平台(V2.5)软件,通过统计报表模块中频次统计分析所有药物的性味归经及使用频次,利用数据分析模块中的关联规则apriori算法、复杂系统熵层次聚类等方法,分析处方中各种药物之间潜在的关联规则,挖掘核心药对,掌握孙增涛教授治疗肺系疾病的用药经验。结果共纳入824张处方,涉及药物385味,其中使用频次≥20%中药120味,使用频次最高药物为桑白皮,其次为前胡、紫菀、款冬花、地骨皮、白前、黄芩等;关联规则显示:支持度≥20%,置信度≥99%,得出核心药物15味,高频药组30组,频次最高为前胡-桑白皮,其次为白前-前胡、地骨皮-桑白皮、紫菀-款冬花;在改进的互信息法基础上,通过复杂系统煽层次聚类分析,设置相关度为6,惩罚度为5,关联系数0.03,挖掘药对21对,再此基础上进一步凝练生成6个新方。结论通过分析发现孙增涛教授治疗肺系疾病经验丰富,治痰为要,调气为先,用药讲究"清、轻、宣、透",组方不乏宣肺、清肺、润肺、养肺之法,寒温并用,辛开苦降,着眼肺脏气机升降,重视中焦脾胃,兼顾五脏六腑。中医传承辅助平台对挖掘名老中医学术经验具有重要价值,可供参考。
Objective Based on the Chinese Medicine Heritage Supporting Platform(v2.5)software,to analyze the medication law of Dr.SUN Zengtao in the treatment of pulmonary diseases.Methods The outpatient cases of Professor SUN Zengtao during 2017.9.1-2018.12.31 were collected,including the main complaint,current medical history,auxiliary examination,tongue and prescription.We entered the data into the Chinese Medicine Heritage Supporting Platform(V2.5)software,and the statistics in the statistical report module was used.We analyzed the taste and frequency of all drugs,and used the association rule apriori algorithm in the data analysis module,complex system entropy hierarchical clustering,etc.to analyze the potential association rules between various drugs in the prescription and mine the core drug pairs.Master SUN Zengtao’s experience on treating pulmonary diseases.Results A total of 824 prescriptions were included,involving 385 kinds of drugs.There were 120 kinds of drugs which use frequency was more than 20%.The highest frequency of use was Sangbaipi(Morus alba L.),followed by Qianhu(Peucedanum praeruptorum Dunn),Ziwan(Aster tataricus),Kuandonghua(Tussilago farfara),Digupi(Lycium chinense),Baiqian(Cynanchum)and Huangqin(Scutellaria baicalensis).Association rules showed the support degree was more than 20%and the confidence was more than 99%.There were 15 core drugs and 30 groups of high-frequency drugs.The drug with the highest frequency was Qianhu(Peucedanum praeruptorum Dunn)-Sangbaipi(Morus alba L.),followed by Baiqian(Cynanchum)-Qianhu(Peucedanum praeruptorum Dunn),Digupi(Lycium chinense)-Sangbaipi(Morus alba L.),Ziwan(Aster tataricus)-Kuandonghua(Tussilago farfara).On the basis of the improved mutual information method,through the hierarchical analysis of complex systems,the correlation degree was set to 6,the penalty degree was 5,the correlation coefficient was 0.03,and the mining drug pair was 21 pairs.On this basis,we further concisely generated 6 new parties.Conclusion Through the analysis,Professor SUN Zengtao has rich experience on treating lung diseases,taking treating phlegm as a priority and regulating Qi as the first,and paying attention to clearing,light,dispersing and letting out.There are methods of dispersing lung,clearing lung,moistening lung and nourishing lung.The treatment should focus on the lung and Qi movement and pay attention to the spleen and stomach.The Chinese medicine inheritance auxiliary platform has important value for excavating the experience of ancient TCM physicians.
