摘要
目的探究血浆NT-proBNP、hs-CRP、Hcy水平、LVEDd、LVEF值与AMI心肌缺血程度及冠脉病变支数的相关性。方法选取2015年6月至2017年6月在我院心内科住院治疗的AMI患者63例作为研究对象,同时选取同期以胸闷胸痛为主要不适主诉入院患者62例作为对照组。观察对照组和不同冠脉病变支数AMI患者血浆NT-proBNP、Hcy、hs-CRP水平、LVEDd值、LVEF变化情况。结果对照组NT-proBNP、hs-CRP、Hcy、LVEDd水平均明显低于AMI组(p<0.05),LVEF值高于AMI组(p<0.05);三支病变组NT-proBNP、hs-CRP、Hcy、LVEDd水平均明显低于单支病变组、双支病变组(p<0.05),LVEF值高于单支病变组、双支病变组(p<0.05)。血浆NT-proBNP水平与糖尿病史、冠状动脉病变支数、hs-CRP水平呈正相关(p<0.05),与LVEF值呈负相关(p<0.05)。结论血浆NT-proBNP、Hcy、hs-CRP水平与AMI心肌缺血程度和冠状动脉病变严重程度密切相关,可作为预测冠状动脉病变严重程度的独立指标,为AMI临床诊断、治疗和预后判断提供依据。
Objective To explore the relationship of plasma NT-pro BNP,hs-CRP,Hcy,LVEDd and LVEF with the severity of myocardial ischemia and the number of coronary lesion of AMI.Methods Sixty-three cases of AMI were chosen as the research object from June 2015 to June 2017 in the heart medicine of our hospital,and 62 cases of chest pain were chosen as the control group at the same time.The level of plasma NT-pro BNP,hs-CRP,Hcy,LVEDd and LVEF of the control group and different number of coronary lesion of AMI patients were compared.Results NT-pro BNP,hs-CRP,Hcy,and LVEDd levels were significantly lower in the control group than in the AMI group(p<0.05)and LVEF values were higher than in the AMI group(p<0.05);NT-pro BNP,hs-CRP,Hcy,and LVEDd levels were significantly lower in the three lesion groups than in the single lesion group and the double lesion group(p<0.05)and LVEF values were higher than in the single lesion group and the double lesion group(p<0.05).Plasma NT-pro BNP levels were positively correlated(p<0.05)with history of diabetes,number of coronary lesion branches,and hs-CRP levels and negatively correlated(p<0.05)with LVEF values.Conclusions There were closely relative of plasma NT-pro BNP,hs-CRP and Hcy with the severity of myocardial ischemia and the number of coronary lesion of AMI,which can be used as independent predictors of the severity of coronary artery lesion and provide basis for clinical diagnosis,treatment and prognosis of AMI patients.
