摘要
目的观察L-阿拉伯糖联合二甲双胍对新诊断2型糖尿病(T2DM)患者胰岛β细胞功能的影响。方法选取2019年1月至2020年6月在内分泌代谢科门诊就诊的新诊断为T2DM的患者53例,按照随机数字表法分为对照组和观察组,对照组27例,观察组26例。对照组为单用二甲双胍,观察组为二甲双胍联合L-阿拉伯糖,比较2组胰岛β细胞功能等方面的差异。结果治疗12周时,2组患者血糖、糖化血红蛋白(Hb A1c)、空腹胰岛素(FINS)和空腹C肽均较治疗前下降(P<0.01);观察组胰岛β细胞功能指数(HOMA-β)较治疗前升高(P<0.01);观察组血糖、Hb A1c、空腹胰岛素(FINS)和胰岛素抵抗指数(HOMA-IR)均低于对照组(P<0.05),经HOMA-IR校正后的胰岛β细胞功能指数(HBCI/IR)和空腹β细胞功能指数(FBCI/IR)、胰岛素敏感指数(ISI)均高于对照组(P<0.01);观察组餐后2 h胰岛素(2h INS)、餐后2 h C肽、HBCI/IR、FBCI/IR、ISI均较对照组显著改善(P<0.05)。结论L-阿拉伯糖与二甲双胍联合应用对降低血糖、餐后胰岛素峰值以及改善胰岛β细胞分泌功能均优于单用二甲双胍。
AIM To investigate the effects of L-arabinose combined with metformin on pancreaticβcell function in patients newly diagnosed with type 2 diabetes mellitus(T2DM).METHODS Newly diagnosed T2DM 53 patients admitted to department of endocrinology from January 2019 to June 2020 were randomly divided into control group(n=27)treated with metformin and observation group(n=26)treated with metformin combined with L-arabinose.Then the pancreaticβcell function were compared between 2 groups.RESULTS At week 12 of treatment,significant decline was noticed in the term of blood glucose,glycosylated hemoglobin(HbA1c),fasting insulin(FINS),and fasting C-peptide(P<0.01)in control group and observation group compared with baseline level.The homeostasis model assessment insulin resistance index of isletβcells(HOMA-β)in the observation group showed significant elevation compared with the baseline level(P<0.01).Compared with the control group,significant decline was noticed in blood glucose,HbA1c and FINS as well as HOMA-IR in the observation group(P<0.05).After adjusting with HOMA-IR in the observation group,there was significant elevation in the HBCI/IR,FBCI/IR and insulin sensitive index(ISI)compared with the control group(P<0.01).There was significant improvement in observation group in the changes of 2hINS,postprandial 2 h C peptide,HBCI/IR,FBCI/IR and ISI compared with control group(P<0.05).CONCLUSION The combination of L-arabinose and metformin is superior to metformin in reducing blood glucose,peak value of postprandial insulin and improving the secretion function of isletβcells.
