摘要
目的探讨基于美国医学物理学家学会(AAPM)293报告的体型特异性辐射剂量估计(SSDE)在成人头部CT扫描中辐射剂量估计应用价值。方法搜集2018年1月至9月在本院Siemens Somatom Definition AS+64排CT机上行头颅CT平扫的762例成人患者,且所有图像均满足诊断要求,记录每例受检者CT容积剂量指数(CTDIvol)。将受检者按头围(HC)大小以四分位法分成3组:A组190例,HC<52.22 cm;B组382例,52.22 cm≤HC≤54.98 cm;C组190例,HC>54.98 cm。通过自主开发软件测得头部扫描中间层面图像的HC、面积及对应平均CT值,并通过AAPM 220和293报告中的计算方法,计算得到水等效直径(WED)、转换系数(f220和f293)、吸收剂量(SSDE220和SSDE293),比较不同头围组中SSDE220和SSDE293差异,以及分析HC与f293的相关性。结果SSDE220(47.14±6.34)mGy较SSDE293(42.20±5.70)mGy平均吸收剂量高估了11.71%;三组不同HC数据中HC、WED、SSDE293和SSDE220的差异均具有统计学意义(F=641.53、503.32、14.54、9.40,P<0.05);CTDIvol差异无统计学意义(F=4.61、P>0.05)。其中,A组SSDE220(47.35±7.43)mGy较SSDE293(42.66±6.67)mGy剂量高估10.99%,B组SSDE220(47.07±6.44)mGy较SSDE293(42.14±5.78)mGy剂量高估11.70%,C组SSDE220(47.06±4.78)mGy较SSDE293(41.86±4.27)mGy剂量高估12.42%;HC与f293具有较高相关性(R2=0.678)。结论在成人头部CT扫描中,相较于AAPM 293报告,应用AAPM 220报告中的估算方法会明显高估受检者的辐射剂量,且HC与f293具有良好的相关性,可利用HC来快速估算SSDE293,优化扫描参数。
Objective To investigate the application value of the American Academy of Medical Physician(AAPM)293 report on size-specific dose estimate(SSDE)in adult head CT scan.Methods A retrospective collection of 762 adults who underwent a routine CT scan at our hospital from January to September 2018 was done,and all images met the diagnostic requirements.The volume CT dose index(CTDIvol)of each subject was recorded.The subjects were divided into 3 groups by the quartile method according to the head circumference(HC)size:group A,HC<52.22 cm,190 cases;group B,52.22 cm≤HC≤54.98 cm,382 cases;group C,HC>54.98 cm,190 cases.The head circumference(HC),area(Area)and corresponding CT values of the mid-level image of the head scan were measured by the self-developed software,and through the calculation method in AAPM220 and 293 reports calculated the water equivalent diameter(WED),conversion factors f220 and f293,and SSDE220 and SSDE293.The differences between SSDE220 and SSDE293 in different HC groups were compared,and the correlation between HC and f293 was analyzed.Results SSDE220(47.14±6.34)is 11.71%overestimated by the average absorbed dose of SSDE293(42.20±5.70);The differences in HC,WED,SSDE293,and SSDE220 among the three groups were statistically significant(t=641.53,503.32,14.54,9.40,P<0.05);the differences in CTDIvol were not statistically significant(t=4.61,P>0.05).Among them,in group A SSDE220(47.35±7.43)was overestimated by 10.99%compared with SSDE293(42.66±6.67),in group B SSDE220(47.07±6.44)was overestimated by 11.70%compared with SSDE293(42.14±5.78)and in group C SSDE220(47.06±4.78)12.42%overestimation of SSDE293(41.86±4.27).Head circumference has a higher correlation with f293(R2=0.678).Conclusion In adult head CT scan,compared with the AAPM report 293,the estimation method in the AAPM report 220 significantly overestimates the radiation dose,and HC has a good correlation with f293,it can be used to quickly estimate SSDE293 and optimize Scan parameters.
