成人肥胖患者血清25羟维生素D3水平及其与糖脂代谢指标相关性研究 被引量:11

Relationship of serum 25-hydroxyvitamin D level with glucose and lipid metabolic parameters in obese adults

摘要 目的分析成人肥胖患者的血清25羟维生素D3[25-(OH)D3]与糖脂代谢指标的相关性。方法采用标准体质指数(BMI)法,筛选单纯性肥胖成人40例,并设置40例同年龄区间、性别的健康非肥胖成人作为正常对照组,检测各组25-(OH)D3水平以及糖脂代谢指标水平。结果与正常对照组相比,肥胖组患者血清25-(OH)D3水平明显低于正常组,差异有统计学意义(P<0.05)。肥胖组空腹血糖(FPG)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、糖化血红蛋白(HbA1c)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)出现不同程度上升,高密度脂蛋白胆固醇(HDL-C)呈一定程度下降,且血清25(OH)D3水平与BMI、FPG、FINS、HOMA-IR、TC、TG、LDL-C呈负相关,与HDL-C呈正相关(P<0.05),与腰围(WC)及HbA1c无明显相关性(P>0.05);肥胖组TG、FPG对血清维生素D水平具有显著负向预测作用。结论与体质指数正常者相比,成人肥胖患者25-(OH)D3水平显著降低,且与FPG、FINS、TC、TG等糖脂代谢参数指标存在显著关联,提示血清25羟维生素D3水平可能在成人肥胖患者糖脂代谢中发挥一定作用。 Objective To analyze the correlations between levels of serum 25-hydroxyvitamin D3[25-(OH)D3]and glucose and lipid metabolic parameters in obese adults.Methods The standard body mass index(BMI)method was used to screen 40 simple obese adults,while another 40 healthy non-obese adults of the same age and gender served as the control group.The levels of 25-(OH)D3 and glycolipid metabolism indexes were measured in each group.Results Compared with the normal control group,the serum level of 25-(OH)D3[(13.51±4.73)ng/ml]in obese adults was significant-1 y lower[(28.98±2.84)ng/ml],and the difference was statistically significant(P<0.05).In obese adults,FPG,FINS,HOMA-IR,TC,TG and LDL-C increased,while HDL-Cdecreased.Moreover,the results of regression analysis showed that serum 25(OH)D3 level was negatively correlated with FINS,HOMA-IR TC,TG and LDL-C,but was positively correlated with HDL-C(r values were-0.872,-0.608,-0.542,-0.347,-0.822,-0.609 and 0.657,respectively,P<0.05).There was no significant correlation between vitamin D and waist circumference or HbAlc.TG and FPG significantly negatively predicted serum VD levels in the obese group.Conclusion The level of serum 25-(OH)D3 in adult obese patients is not only significantly lower than that in adults with normal body mass,but significantly correlated with glucose and lipid parameters,suggesting that serum vitamin D level may may play a role in glucose and lipid metabolism in obese adults.
