摘要
目的探讨老年人群中不同肥胖类型对高尿酸血症(Hyperuricemia,HUA)的影响。方法选择常州市武进区2021年纳入基本公卫服务管理≥65岁人群104433人,通过问卷调查、体格检查与实验室检查获取信息,根据体质量指数(BMI)和腰高比(WHtR)将该人群分为非肥胖(normal weight,NW)、单纯中心性肥胖(obesity,OB)、单纯全身性肥胖(normal weight with central obesity,NWCO)、复合性肥胖(obesity with central obesity,OBCO)4种类型。采用性别分层单因素分析、多因素logistic回归分析不同肥胖类型对HUA发病的影响。结果共纳入100015人,HUA患病率14.44%,男性患病率为21.94%,女性为7.69%,差异有统计学意义(χ^(2)=4102.14,P<0.01)。NWCO、OB、OBCO人群,男性患病率分别为31.78%、1.49%、36.43%,女性分别为40.84%、0.97%、38.65%。不论男女,NWCO、OB、OBCO均会增加HUA患病风险(OR值均>1,P值均<0.01)。结论单纯中心性肥胖、复合性肥胖是≥65岁人群中HUA发生的独立危险因素。可以通过BMI和WHtR筛查老年人群高尿酸血症高危人群,尽早开展不同性别高尿酸血症危险因素的差异化健康管理。
Objective To examine the effect of obesity type on risk of hyperuricemia among the elderly populations,so as to provide insights into formulation of the preventive strategy for hyperuricemia.Methods A total of 104433 elderly residents at ages of 65 years and older that were included in the basic public health service program in Wujin District,Changzhou City,2021 were recruited,and participants’demographic and clinical features were collected using questionnaire surveys,physical examinations and laboratory testing.All subjects were classified into the non-obesity(NO)group,central obesity(OB)group,generalized obesity(GO)group and compound obesity(CO)group according to body mass index(BMI)and waist-to-height ratio(WHtR).The effect of obesity type on the risk of hyperuricemia was examined using gender-stratified univariate analysis and multivariate logistic regression analysis.Results A total of 100015 elderly populations were enrolled,and the overall prevalence of hyperuricemia was 14.44%,with higher prevalence among men than among women(21.94%vs.7.69%;χ^(2)=4102.14,P<0.01).The prevalence of hyperuricemia was 31.78%,1.49%and 36.43%among men,and 40.84%,0.97%and 38.65%among women in the GO,OB and CO groups,respectively.In addition,GO,OB and CO all increased the risk of hyperuricemia regardless of gender(all OR>1,all P values<0.01).Conclusions OB and CO are independent risk factors of hyperuricemia among the elderly populations at ages of 65 years and older.The elderly populations at a high risk of hyperuricemia are recommended for screening through measurement of BMI and WHtR,and personalized health management targeting gender-specific risk factors of hyperuricemia is required as early as possible.
