摘要
目的观察清肺化痰方对慢性阻塞性肺病急性加重期患者的治疗效果。方法将我院72例慢性阻塞性肺病急性加重期患者随机分为观察组与对照组各36例,对照组给予控制感染、平喘、呼吸支持、解痉等常规西医治疗,观察组在对照组基础上加服清肺化痰方治疗,比较两组患者治疗总有效率、肺功能指标、血气指标、血清白介素-6(IL-6)和白介素-10(IL-10)水平、咳嗽缓解时间、喘鸣消失时间及住院时间。结果治疗后观察组总有效率高于对照组(P<0.05);观察组第1s用力呼气量(FEV1%)、FEV1占用力肺活量百分比(FEV1/FVC)、动脉血氧分压(PaO_(2))均显著高于对照组(P<0.05)动脉血二氧化碳分压(PaCO_(2))显著低于对照组(P<0.05);观察组血清IL-6、IL-10水平显著高于对照组(P<0.05);观察组咳嗽缓解时间、喘鸣消失时间和住院时间均显著短于对照组(P<0.05)。结论常规治疗联合清肺化痰方可显著提高慢性阻塞性肺病急性加重期患者的临床疗效,改善患者肺功能、血气指标和血清细胞因子水平,缩短临床症状消失时间,具有较好的治疗效果。
Objective To observe the therapeutic effect of Qingfei Huatan Decoction on patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods Seventy-two patients with acute exacerbation of AECOPD were randomly divided into observation group and control group,36 cases each.Control group was treated with routine western medicine,such as infection control,asthma relief,respiratory support and spasmolysis,while observation group was treated with Qingfei Huatan Decoction on the basis of control group.The total effective rate,pulmonary function index,blood gas index,serum interleukin-6(IL-6),IL-10,cough relief time,wheezing disappearance time and hospitalization time were compared between the two groups.Results After treatment,the clinical efficacy of observation group was significantly better than that of control group(P<0.05),the forced expiratory volume in 1s(FEV1%),FEV1 occupied vital capacity percentage(FEV1/FVC),arterial partial pressure of oxygen(PaO_(2))of observation group was significantly higher than that of control group(P<0.05),serum IL-6 level in observation group was significantly lower than that in control group(P<0.05)while IL-10 level was significantly higher(P<0.05);the time of cough relief,wheezing disappearance and hospitalization in observation group were significantly shorter than those in control group(P<0.05).Conclusion Routine treatment combined with Qingfei Huatan Decoction can significantly improve the clinical efficacy of patients with AECOPD,improve lung function and blood gas index,improve serum cytokine level,shorten the time of clinical symptoms disappear with better therapeutic effect.
