摘要
目的对比生物陶瓷iRoot BP Plus和三氧化钙无机聚合物(mineral trioxide aggregate,MTA)在龋源性露髓恒牙活髓保存术中的应用效果。方法选取我院2019年12月~2021年4月85例龋源性露髓恒牙患者(患牙117颗)作为研究对象。采用电脑随机数法将其分为A组和B组,其中A组42例(患牙51颗)予以MTA盖髓,B组43例(患牙66颗)予以iRoot BP Plus盖髓。比较2组患者治疗6个月后的患牙影像学恢复效果、牙髓活力、术后不同时间点疼痛程度及碱性磷酸酶(alkaline phosphatase,AKP)、血清骨钙素(bone gla-protein,BGP)水平,并记录不良反应。结果B组术后治疗有效率优于A组(P<0.05);A组牙髓电活力测验优良率(80.39%)低于B组(93.94%)(P<0.05);2组患者术后疼痛程度、时间点及交互作用差异无统计学意义(P>0.05);A组术后0.5、1、4 h的疼痛程度与B组比较,差异无统计学意义(P>0.05);2组术后4 h的疼痛程度均低于术后0.5 h(P<0.05);2组患者术后血清AKP、BGP水平均降低,B组均低于A组(P<0.05);2组患者不良反应发生率比较,无统计学意义(P>0.05)。结论生物陶瓷iRoot BP Plus应用于龋源性露髓恒牙活髓保存术更有助于改善牙髓活力,整体治疗效果良好。
Objective To compare the effect of bioceramic iRoot BP Plus and mineral trioxide aggregate(MTA)in vital pulp preservation of carious exposed permanent teeth.Methods From December 2019 to April 2021,85 carious exposed permanent teeth patients(117 affected teeth)were selected as the study subjects.The patients were divided into group A and group B by computer random number method.In group A,42 patients(51 affected teeth)received MTA pulp capping,and in group B,43 patients(66 affected teeth)received iRoot BP Plus pulp capping.After 6 months of treatment,the imaging recovery effect,pulp vitality and pain degree at different postoperative time points,serum level of alkaline phosphatase(AKP)and serum osteocalcin(BGP)were compared between the two groups,and the adverse reactions were recorded.Results The postoperative effective rate of group B was better than group A(P<0.05).The excellent and good rate of dental pulp electroactivity test in group A(80.39%)was lower than that in group B(93.94%)(P<0.05).There were no significant differences in postoperative pain degree,time points and interaction between the two groups(P>0.05).There was no significant difference in pain degree at 0.5,1 and 4 h after operation between the two groups(P>0.05).The pain degree of both groups at 4 h after surgery was lower than 0.5 h after surgery(P<0.05).The serum AKP and BGP levels of the two groups were decreased after operation,and the levels of group B were lower than those of group A(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The application of bioceramic iroot BP plus in the pulp preservation of carious exposed permanent teeth is more helpful to improve the pulp vitality,and the overall treatment effect is good.
