摘要
目的探讨经直肠前列腺癌典型超声表现与特征预测前列腺癌的准确性。方法选取137例PSA>10.0 ng/mL患者均行经直肠前列腺饱和穿刺术,于穿刺前所有患者均行经直肠超声检查,采集前列腺横切面及矢状切面图像,观察前列腺外周带是否多发微钙化、外周带回声是否显著减低、内外腺分界清晰与否、有无前列腺结节。结果经直肠超声检查前列腺137例,穿刺病理确诊腺癌88例,其中①外周带细点状钙化23例,恶性23例,敏感性26.1%、特异性100%、准确性52.5%;②外周带实质回声显著减低60例,恶性56例,敏感性63.6%、特异性91.8%、准确性73.7%;③内外腺结构分界不清57例,恶性55例,敏感性62.5%、特异性95.9%、准确性74.4%;④外周带结节7例,恶性2例,敏感性2.28%、特异性89.8%、准确性33.5%。结论经直肠超声影像学检查对于诊断前列腺癌具有很高的应用价值。
Objective To investigate the accuracy of ultrasonographic findings and characteristics in the prediction of prostate cancer.Methods One hundred and thirty seven patients with PSA>10.0 ng/mL were selected for transrectal prostate saturation puncture.Before puncture,transrectal ultrasonography was performed in all patients to collect cross-sectional and sagittal images of the prostate,and observed whether the peripheral zone of the prostate had more microcalcification,whether the echo of the peripheral zone had decreased significantly,whether the boundary between the internal and external glands was clear or not,and whether there were prostate nodules.Results One hundred and thirty seven cases of prostate were examined by transrectal ultrasound and eighty-eight cases of adenocarcinoma were diagnosed by pathology.Among them Peripheral zone fine spot calcification twenty-three cases,malignant twenty-three cases,with sensitivity of 26.1%,specificity of 100%,accuracy of 52.5%;Peripheral parenchymal echo decreased significantly in sixty cases and malignant in fifty-six cases,with sensitivity of 63.6%,specificity of 91.8%,accuracy of 73.7%;The boundary of the internal and external gland structures was unclear in fifty-seven cases and malignant in fifty-five cases,with sensitivity of 62.5%,specificity of 95.9%,accuracy of 74.4%;Peripheral nodules were found in seven cases and malignant nodules in two cases,with sensitivity of 2.28%,specificity of 89.8%,accuracy of 33.5%.Conclusion Transrectal ultrasonography has great value in the diagnosis of prostate cancer.
