摘要
目的探讨导师制联合进阶式教学法对临床专业来源的麻醉科住院医师规范化培训(简称住培)成效。方法回顾性分析2014年至2020年在丽水市中心医院麻醉科专业基地21例临床专业毕业生作为观察组,将19例麻醉专业本科毕业生作为对照组。对比两组参加住培的人员在4个培训阶段、3个年度学习的理论与实践考试成绩,以及在国家医师资格考试和住培结业考核中的通过率。结果入职时观察组的实践技能、综合理论成绩与对照组比较差异有统计学意义(P<0.05)。在住培第一阶段和第一学年的实践技能与综合理论考试中,以及住培第二阶段和第二学年的实践技能考试成绩中,观察组与对照组比较差异有统计学意义(P<0.05)。但在住培第三、四阶段以及第三学年的实践技能考试和综合理论考试成绩中,观察组与对照组比较差异均无统计学意义(P>0.05)。首次医师资格考试实践技能考试和住培结业实践技能考试中,观察组与对照组比较差异有统计学意义(P<0.05)。在综合理论考试通过率上观察组与对照组的通过率均为100%。结论采用导师制联合进阶式住培教学法可以促进临床专业来源的麻醉科住院医师的能力成长,经3年住培后其麻醉学知识掌握程度与麻醉专业来源的临床胜任能力无明显差异。
Objective To explore the effect of the tutorial system combined with advanced teaching method on the standardized residency training for clinical or anesthesia undergraduate students in anesthesiology professional bases.Methods Retrospective analysis was performed.Twenty-one clinical undergraduate students enrolled in Lishui Central Hospital from 2014 to 2020 were included in the study group,and 19 anesthesia undergraduate students were selected as the control group.The results of theory and practice tests in 4 training stages and 3 years of study were compared between the two groups,as well as the passing rates in the state medical lisensing examination and residency training completion tests.Results There were statistically significant differences between the study group and the control group in practical skills and comprehensive theoretical performance(P<0.05).There were significant differences between the study group and the control group in the practical skills and comprehensive theory tests of the first stage and the first academic year of residency training(P<0.05).In the practical skill tests of the second stage and the second academic year of residency training,there were statistically significant differences between the study group and the control group(P<0.05).However,there were no significant differences between the study group and the control group in the scores of practical skill tests and comprehensive theory tests in the third and fourth stages and the third academic year of residency training(P>0.05).There were statistically significant differences between the study group and the control group in the first-time practical skill tests in practitioners and the residency training completion practical skill test(P<0.05).In the comprehensive theory examination,the passing rates of both the study group and the control group were 100%.Conclusion The combination of the tutor system and the advanced residency training teaching method can promote the ability growth of the clinical majors after entering the anesthesia post,and there is no significant difference between the degree of anesthesia knowledge mastery and the clinical competence of the anesthesia majors after the 3-year residency training.
