摘要
目的分析药师窗口调配发药差错及原因,提升儿童药房药学服务质量。方法对2018年1月―2019年12月儿童西药房220例发药差错进行分析,查找原因并制定相应防范措施。结果220例差错中,药品品种差错65例,药品数量差错60例,用法用量差错35例,剂型与规格差错32例,医生处方差错18例,其他差错10例。结论加强药师业务培训,采取必要的措施防止差错发生,充分发挥合理用药系统对不合理处方的拦截作用,规范管理,提高药师对儿童用药沟通重要性的认识,促进儿童合理用药。
Objective To analyze the causes of drug distribution errors at the pharmacist window in order to improve the quality of pharmaceutical services in pediatric pharmacies.Methods 220 cases of drug distribution errors in paediatric pharmacies of western medicine from January 2018 to December 2019 were analyzed.The reasons were found and corresponding preventive measures were formulated.Results Among 220 cases of errors,there were 65 cases of drug variety errors,60 cases of drug quantity errors,35 cases of usage and dosage errors,32 cases of dosage forms and specifications errors,18 cases of doctor prescription errors,and 10 cases of other errors.Conclusion Measures should be taken to promote rational drug use in paediatric outpatient department such as strengthening the professional training of pharmacists,taking necessary measures to prevent the occurrence of errors,giving full play to the role of rational drug use system in intercepting unreasonable prescriptions,standardizing management,and improving pharmacists’awareness of the importance of communication about children’s drug use.
