摘要
目的比较白内障超声乳化吸除术前角膜内皮细胞密度(endothelial cell density,ECD)≤2000个/mm2(低ECD组)与ECD>2000个/mm2(正常组)两组角膜内皮细胞经该手术后的改变.方法收集术前ECD≤2000个/mm2及同时段术前ECD>2000个/mm2患者各59例.所有患者均行白内障超声乳化吸除联合人工晶状体植入术,定期随访(术后1、3月,1年)共3次,分别记录ECD、中央角膜厚度(central corneal thickness,CCT),比较两组患者ECD、CCT、ECD降低率及CCT增加率.结果术前低ECD组ECD明显小于正常组,ECD随时间下降(P<0.05);CCT组间比较差异及随时间变化趋势无统计学意义(P>0.05),CCT先增加后逐渐恢复至术前水平,术前低ECD组CCT大于正常组.两组在术后各个时间点分别与术前比较,ECD下降值、下降率与CCT增加值、增加率组间差异无统计学意义(P>0.05).随访期间所有患者无大泡性角膜病变发生.结论白内障超声乳化吸除联合人工晶状体植入术后,术前低ECD组患者的角膜内皮细胞损伤程度与术前ECD>2000个/mm2患者基本相同,即对低ECD组患者单独行白内障超声乳化吸除术可获得安全、良好的手术效果.
Objective To compare ECD,CCT between preoperative ECD less thanand more than 2000/mm2 group after phacoemulsification.Methods The 59 patients experiencedphacoemulsification and IOL implantationin each group were collected.ECD and CCT were recorded at 1 month,3,12 months postoperatively and compared in each groups.Results ECD in low ECD group was less than normal grouppreoperatively and decreased with time(P<0.05).Therewas no statistical significance in comparison between groups and time variation in CCT groups.CCT increased and then restored to the baseline.CCT was more in low ECD group.No statistical difference in cell loss,CCT increase was found between groups.No bullous keratopathy occurred in all patients during the follow-up period.Conclusions Endothelial damage after phacoemulsificationin low ECD is comparable to that in control eyes,indicatingthat phacoemulsification performed in low ECD eyes could get safe andgreatresults.
