摘要
目的分析振动排痰机排痰在ICU呼吸机相关性肺炎患者中的应用效果.方法选取2018年2月-2020年11月期间在我院诊治的78例icu呼吸机相关性肺炎患者为研究对象,随机分为对照组和观察组,各组39例.对照组采用人工叩背排痰,观察组振动排痰机排痰,观察比较临床治疗指标、呼吸功能指标[气道峰值吸气压力(PIP)、气道阻力(RAW)]、急性生理和慢性健康状况评分(APACHEⅡ)、临床肺部感染(CPIS)评分.结果观察组每日排痰量高于对照组,抗生素使用时间短于对照组(P<0.05);治疗后,2组PIP、RAW均低于治疗前,且观察组低于对照组(P<0.05);治疗后3d、7d,2组APACHEⅡ评分均低于治疗前,且观察组低于对照组(P<0.05);治疗后3d、7d,2组CPIS评分均低于治疗前,且观察组低于对照组(P<0.05).结论振动排痰机排痰在icu呼吸机相关性肺炎患者具有显著的应用效果,可显著改善呼吸功能,增加每日排痰量,缩短抗生素使用时间,降低APACHEⅡ和CPIS评分,具有更显著的应用优势.
Objective To analyze the effect of vibrating sputum elimination machine in ICU patients with ventilator-associated pneumonia.Methods 78 patients with ventilator-associated pneumonia in ICU from February 2018 to November 2020 were randomly divided into control group(n=39)and observation group(n=39).The patients in the control group were treated with manual back percussion,while the patients in the observation group were treated with vibrating expectoration machine,the clinical therapeutic index,respiratory function index[peak inspiratory pressure(Pip),airway resistance(RAW)],acute physiology and chronic health status score(Apache II),and clinical pulmonary infection(CPIS)were observed and compared.Results Compared with the control group,the amount of sputum discharged per day and the time of using antibiotics in the observation group were significantly shorter(P<0.05),Pip and RAW in both groups were significantly lower after treatment than before treatment,and those in the observation group were significantly lower than those in the control group(P<0.05),and the levels of PIP and RAW in the observation group were significantly lower than those in the control group(P<0.05),the scores of Apache II in both groups were lower than those before treatment,and the scores of CPIS in the observation group were lower than those in the control group(P<0.05),and the scores of CPIS in both groups were lower than those before treatment on the 3rd and 7th day after treatment,and the scores of the observation group were lower than those in the control group(P<0.05).Conclusion Vibration sputum elimination machine has a significant effect in ICU patients with ventilator-associated pneumonia,can significantly improve respiratory function,increase the daily output of sputum,shorten the use of antibiotics,reducing Apache II and CPIS scores has more significant application advantages.
