摘要
目的:探讨急诊重症监护病房(EICU)急性脑出血患者压疮发生的影响因素.方法:采用整群抽样法选取2020 年 1 月至 2022 年 6 月就诊于本院EICU的 758 例急性脑出血患者为研究对象.将其中发生压疮的 15 例患者纳入压疮组,未发生压疮的 743 例患者纳入非压疮组.收集、整理两组患者的临床资料,分析EICU急性脑出血患者压疮发生的影响因素.结果:单因素分析结果显示,血清蛋白水平、是否使用镇痛镇静药、是否使用呼吸机、血红蛋白水平、入院时改良Rankin量表(mRS)评分与EICU急性脑出血患者压疮发生有关(P<0.05);多因素分析结果显示,血清蛋白<25g/L、使用镇痛镇静药、使用呼吸机、血红蛋白<135g/L、入院时mRS评分>3 分是EICU急性脑出血患者压疮发生的危险因素(P<0.05).结论:血清蛋白水平、是否使用镇痛镇静药、是否使用呼吸机、血红蛋白水平、入院时mRS评分是EICU急性脑出血患者发生压疮的危险因素.
Objective:To investigate the influencing factors of pressure ulcers in patients with acute cerebral hemorrhage in emergency intensive care unit(EICU).Methods:Seven hundred and fifty-eight patients who suffered from acute cerebral hemorrhage in EICU of our hospital from January 2020 to June 2022 were selected by cluster sampling method.Among them,15 patients with pressure ulcers were included in the pressure ulcers group,and 743 patients without pressure ulcers were included in the non-pressure ulcers group.The clinical data of patients were collected and sorted out,and the influencing factors of pressure ulcers in patients with acute cerebral hemorrhage in EICU were analyzed.Results:Univariate analysis showed that serum protein level,the use of analgesic and sedative drugs,the use of ventilator,hemoglobin level,modified Rankin scale(mRS)score on admission were associated with pressure ulcers in patients with acute cerebral hemorrhage in EICU,and the differences were statistically significant(P<0.05).Multivariate analysis showed that serum protein<25g/L,use of analgesic and sedative drugs,use of ventilator,hemoglobin<135g/L,mRS score>3 at admission were high risk factors for pressure ulcers in patients with acute cerebral hemorrhage in EICU(P<0.05).Conclusions:The influencing factors of pressure ulcers in patients with acute cerebral hemorrhage in EICU are serum protein level,the use of analgesic and sedative drugs,the use of ventilators,hemoglobin level,mRS score at admission.
