摘要
目的:比较药物涂层球囊(DCB)、药物洗脱支架(DES)治疗急性冠状动脉综合征(ACS)的临床应用疗效.方法:选取本院于 2020 年 1 月至 2022 年 7 月收治的ACS患者 104 例,按照治疗方案的不同分为观察组(n =48,DCB)和对照组(n =56,DES).比较两组患者临床资料、靶血管最小管腔直径、晚期管腔丢失指标、冠状动脉造影特点及不良心血管事件.结果:观察组的术后即刻最小管腔直径小于对照组,随访时最小管腔直径大于对照组(P<0.05).观察组冠状动脉单支病变率、左前降支病变率及迂曲病变率低于对照组(P<0.05).两组患者术后 6 个月复查发现,观察组靶血管血运重建率及非致死性心肌梗死、心源性死亡发生率低于对照组(P<0.05).结论:DCB更适合代替DES植入治疗ACS.DCB可减少管腔狭窄程度,加大最小管腔直径,降低靶血管血运重建率及主要不良心血管事件风险,更具有临床应用价值.
Objective:To study the clinical application of drug-coated balloon(DCB)and drug-eluting stent(DES)in the treatment of acute coronary syndrome(ACS).Methods:One hundred and four patients with ACS admitted to our hospital from January 2020 to July 2022 were selected into this study,and according to different therapeutic regimens,they were divided into observation group(n=48,DCB)and control group(n=56,DES).The clinical data,minimum target vessel lumen diameter,late lumen loss index,coronary angiography characteristics and adverse cardiovascular events were compared between the two groups.Results:The minimum lumen diameter immediately after surgery in the observation group was smaller than that in the control group,and during follow-up it was larger than that in the control group(P<0.05).The rate of single coronary artery lesion,left anterior descending artery lesion and tortuous lesion in the observation group were lower than those in the control group(P<0.05).The re-examination results after 6 months of operation showed that the revascularization rate of target vessels and the incidence of non-fatal myocardial infarction and cardiac death in the observation group were lower than those in the control group(P<0.05).Conclusion:DCB can reduce the degree of lumen stenosis,increase the minimum lumen diameter,decrease the rate of target vessel revascularization and the risk of major adverse cardiovascular events,which is more suitable to replace DES implantation and has better clinical value in the treatment of ACS.
