摘要
目的探究显微镜辅助经侧裂入路手术对高血压脑出血(HICH)患者的影响.方法选取该院2021年4月—2023年4月收治的86例HICH患者为研究对象,按随机数字表法分为对照组与观察组,各43例.对照组行经颞叶皮质入路手术,观察组行显微镜辅助经侧裂入路手术,术后观察1个月.对比两组患者的手术情况、神经因子水平及并发症发生情况.结果观察组手术时间为(83.25±4.49)min,短于对照组的(109.96±5.31)min,血肿清除率为(91.25±3.63)%,高于对照组的(80.42±2.57)%,并发症发生率为4.65%,低于对照组的18.60%,组间差异有统计学意义(P﹤0.05).术后,神经元特异性烯醇化酶为(10.38±1.24)μg/mL,胶质纤维酸性蛋白为(3.24±0.40)pg/mL,S-100β蛋白为(0.08±0.01)ng/mL,均低于对照组的(13.21±1.59)μg/mL、(4.01±0.59)pg/mL、(0.21±0.03)ng/mL,组间差异有统计学意义(P﹤0.05).结论显微镜辅助经侧裂入路手术可清除HICH患者血肿,降低神经因子水平,且并发症少.
Objective To explore the effect of microscope-assisted lateral fissure approach surgery on patients with hypertensive intracerebral hemorrhage(HICH).Methods 86 patients with HICH admitted to the hospital from April 2021 to April 2023 were selected as the study objects,and were divided into a control group and an observation group according to random number table method,with 43 cases in each group.The control group underwent transtemporal cortex approach,and the observation group underwent microscopy-assisted translateral cleft approach.The operation was observed for 1 month after operation.The operation status,the level of neurofactors and the occurrence of complications were compared between the two groups.Results The operation time in the observation group was(83.25±4.49)min,shorter than(109.96±5.31)min in the control group,the clearance rate of hematoma was(91.25±3.63)%,higher than(80.42±2.57)%in the control group,the incidence of complications was 4.65%,lower than 18.60%in the control group,the differences between groups were statistically significant(P﹤0.05).After operation,neuron-specific enolase was(10.38±1.24)μg/mL,glial fibrillary acidic protein was(3.24±0.40)pg/mL,and S-100βprotein was(0.08±0.01)ng/mL,they were lower than(13.21±1.59)μg/mL,(4.01±0.59)pg/mL,(0.21±0.03)ng/mL of the control group,and there were statistical significance between groups(P﹤0.05).Conclusion Microscopic assisted surgery through the lateral fissure approach can clear hematoma in HICH patients,reduce nerve factor levels,and have fewer complications.
