摘要
目的分析对比无头双加压螺钉内固定与空心螺钉内固定术在踝关节骨折患者中的效果。方法选取2021年2月—2023年2月该院收治的80例踝关节骨折患者为研究对象,按随机数字表法将其分为对照组和观察组,各40例。对照组行空心螺钉内固定术,观察组行无头双加压螺钉内固定术,观察到术后6个月。对比两组围术期指标、踝关节活动度、踝关节功能及并发症发生情况。结果两组术中出血量、手术时间相比,组间差异无统计学意义(P>0.05);观察组骨折愈合时间为(10.53±1.88)周,完全负重时间为(12.67±2.15)周,均短于对照组的(13.69±2.46)周、(14.21±2.58)周,并发症发生率为5.00%,低于对照组的20.00%,组间差异有统计学意义(P<0.05);术前,两组踝关节屈、伸、旋前、旋后活动度与美国足踝外科学协会(AOFAS)踝-后足功能评分相比,组间差异无统计学意义(P>0.05);术后,观察组踝关节屈、伸、旋前、旋后活动度分别为(28.64±3.20)°、(8.97±2.31)°、(19.68±3.05)°、(16.84±3.13)°,均大于对照组的(24.35±2.71)°、(7.25±1.69)°、(17.36±2.48)°、(14.25±2.29)°,AOFAS踝-后足功能评分为(84.36±6.54)分,高于对照组的(77.36±5.89)分,组间差异有统计学意义(P<0.05)。结论无头双加压螺钉内固定术更能够促进踝关节骨折患者骨折愈合,提升踝关节活动度,改善踝关节功能,且并发症少,临床可进行推广与应用。
Objective To analyze and compare the effect of headless double compression screw internal fixation and hollow screw internal fixation in patients with ankle fracture.Methods 80 patients with ankle fracture admitted to the hospital from February 2021 to February 2023 were selected as the research objects.According to the random number table method,they were divided into a control group and an observation group,with 40 cases in each group.The control group was treated with hollow screw internal fixation,and the observation group was treated with headless double compression screw internal fixation.The patients were observed for 6 months after operation.The perioperative indexes,ankle range of motion,ankle function and occurrence of complications were compared between the two groups.Results There was no significant difference in intraoperative blood loss and operation time between the two groups(P>0.05);the fracture healing time and full weight-bearing time of the observation group were(10.53±1.88)weeks and(12.67±2.15)weeks,which were shorter than(13.69±2.46)weeks and(14.21±2.58)weeks of the control group,and the incidence of complications was 5.00%,which was lower than 20.00%of the control group,and the differences between the two groups were statistically significant(P<0.05);there were no significant differences in the ankle flexion,extension,pronation,supination range of motion and the American Orthopaedic Foot and Ankle Society(AOFAS)ankle-hindfoot function score between the two groups before operation(P>0.05);after operation,the ankle flexion,extension,pronation and supination activities of the observation group were(28.64±3.20)°,(8.97±2.31)°,(19.68±3.05)°and(16.84±3.13)°,respectively,which were greater than(24.35±2.71)°,(7.25±1.69),(17.36±2.48)°,(14.25±2.29)°of the control group,the AOFAS ankle-hindfoot function score was(84.36±6.54)points,which was higher than(77.36±5.89)points in the control group,the differences between the two groups were statistically significant(P<0.05).Conclusion The headless double compression screw internal fixation can promote the fracture healing of patients with ankle fracture,improve the range of motion of the ankle joint,improve the function of the ankle joint,and has fewer complications,which is worthy of clinical promotion and application.
