摘要
目的分析对复杂性肛瘘患者行保留括约肌挂线法或传统管切除术治疗的疗效及安全性.方法选取2022年4月—2023年4月我院收治的70例复杂性肛瘘患者为研究对象,按随机数字表法将其分为对照组与观察组,各35例.对照组采用传统瘘管切除术治疗,观察组采用保留括约肌挂线法治疗.比较两组临床疗效﹑临床指标﹑肛门功能及并发症发生情况.结果治疗后,观察组治疗总有效率94.29%高于对照组的77.14%,观察组术中失血量为(30.54±8.65)mL,少于对照组,创伤面积为(2.01±1.02)cm^(2),小于对照组,手术时间为(40.16±9.42)min,创面愈合时间为(21.46±3.65)d,住院时间为(18.26±3.41)d,均短于对照组,肛管静息压为(10.03±1.11)kPa,肛管最大收缩压为(13.24±1.10)kPa,均高于对照组,肛管高压区长度为(4.53±0.74)cm,长于对照组,并发症发生率为5.72%,低于对照组的22.86%,组间差异有统计学意义(P<0.05).结论对复杂性肛瘘患者行保留括约肌挂线法治疗效果更佳,可缩短创面愈合时间,改善肛门功能,降低并发症发生率,安全可靠.
Objective To analyze the efficacy and safety of sphincter retention and traditional tubectomy for patients with complex anal fistula.Methods 70 patients with complex anal fistula admitted to our hospital from April 2022 to April 2023 were selected as the research objects.According to the random number table method,they were divided into a control group and an observation group,with 35 cases in each group.The control group was treated with traditional fistulectomy,and the observation group was treated with sphincter-preserving seton method.The clinical efficacy,clinical indicators,anal function and complications were compared between the two groups.Results After treatment,the total effective rate of the observation group was 94.29%,which was higher than 77.14%of the control group.The intraoperative blood loss of the observation group was(30.54±8.65)mL,which was less than that of the control group,and the wound area was(2.01±1.02)cm^(2),which was less than that of the control group.The operation time was(40.16±9.42)min,the wound healing time was(21.46±3.65)d,and the hospitalization time was(18.26±3.41)d,which were shorter than those of the control group.Anal resting pressure was(10.03±1.11)kPa,anal maximum systolic pressure was(13.24±1.10)kPa,which were higher than those in the control group,anal high pressure zone length was(4.53±0.74)cm,which was longer than that in the control group,the incidence of complications was 5.72%in the control group,which was less than 22.86%in the control group,and the difference between the two groups was statistically significant(P<0.05).Conclusion The effect of sphincter-preserving seton method in the treatment of complex anal fistula is better,which can reduce the wound healing time,improve anal function,reduce the incidence of complications,and is safe and reliable.
