摘要
目的探究带状疱疹后遗神经痛(PHN)患者采用利多卡因凝胶贴膏联合椎旁神经阻滞治疗的效果。方法回顾性分析本院2020年1月—2022年11月收治的58例PHN患者的临床资料,按照治疗方法不同分为两组。将采用超声引导下椎旁神经阻滞治疗的29例患者设为对照组,将采用超声引导下椎旁神经阻滞联合利多卡因凝胶贴膏治疗的29例患者设为观察组,均治疗4周。比较两组患者的疼痛程度、临床效果、睡眠质量及不良反应发生情况。结果观察组的治疗总有效率为93.10%,高于对照组的72.41%,差异有统计学意义(P<0.05)。治疗1、2、3、4周后,观察组的疼痛视觉模拟评分分别为(5.21±0.83)分、(3.91±0.52)分、(3.19±0.36)分、(2.63±0.16)分,均低于对照组,组间差异有统计学意义(P<0.05)。治疗1、2、3、4周后,观察组的匹兹堡睡眠质量指数量表评分分别为(13.10±1.06)分、(11.10±1.02)分、(9.14±0.76)分、(6.32±0.58)分,均低于对照组,组间差异有统计学意义(P<0.05)。两组均未出现明显不良反应。结论采用利多卡因凝胶贴膏联合椎旁神经阻滞治疗PHN患者可有效减轻疼痛,改善其睡眠质量,且安全可靠。
Objective To investigate the effect of lidocaine gel paste combined with paravertebral nerve block in patients with postherpetic neuralgia(PHN).Methods The clinical data of 58 patients with PHN admitted to the hospital from January 2020 to November 2022 were retrospectively analyzed and were divided into two groups according to different treatment methods.29 patients treated with ultrasond-guided paravertebral nerve block were set as the control group,and 29 patients treated with ultrasond-guided paravertebral nerve block combined with lidocaine gel paste were set as the observation group.Both groups were treated for 4 weeks.The pain degree,clinical effect,sleep quality and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 93.10%,which was higher than 72.41%of the control group,the difference was statistically significant(P<0.05).After 1,2,3 and 4 weeks of treatment,the pain visual analogue scale scores in the observation group were(5.21±0.83)points,(3.91±0.52)points,(3.19±0.36)points and(2.63±0.16)points,respectively,which were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).After 1,2,3 and 4 weeks of treatment,the Pittsburgh sleep quality index scores of the observation group were(13.10±1.06)points,(11.10±1.02)points,(9.14±0.76)points and(6.32±0.58)points,respectively,which were lower than those of the control group,and the differences between the groups were statistically significant(P<0.05).There were no obvious adverse reactions in both groups.Conclusion Lidocaine gel paste combined with paravertebral nerve block can effectively relieve pain and improve sleep quality in patients with PHN,and it is safe and reliable.
