摘要
目的分析替普瑞酮改良四联疗法在幽门螺杆菌(Hp)阳性胃溃疡患者中的应用效果.方法选择2020年9月—2022年7月该院收治的72例Hp阳性胃溃疡患者为研究对象,按随机数字表法分为两组.对照组(n=36)采用铋剂四联疗法,观察组(n=36)采用替普瑞酮改良四联疗法,均连续治疗4周.对比分析两组胃功能、胃黏膜形态、血清炎症因子水平、临床疗效、Hp根除率以及不良反应.结果治疗后,观察组胃蛋白酶原(PG)Ⅰ水平(116.91±12.46)μg/L、PGⅡ水平(5.29±1.07)μg/L及治疗总有效率94.44%、Hp根除率94.44%均高于对照组的(104.53±8.27)μg/L、(4.63±1.02)μg/L、75.00%、77.78%,黏膜形态评分(0.42±0.10)分、黏膜厚度评分(0.30±0.09)分、炎性细胞浸润程度评分(0.60±0.25)分、腺体密度评分(0.38±0.07)分及C反应蛋白(4.28±0.51)mg/L、白细胞介素(IL)-1(19.35±3.67)ng/L、IL-6(48.38±5.12)ng/L均低于对照组的(0.65±0.19)分、(0.48±0.13)分、(0.78±0.29)分、(0.61±0.18)分、(6.03±0.78)mg/L、(30.17±4.28)ng/L、(71.56±9.24)ng/L,组间差异有统计学意义(P<0.05).两组不良反应发生率相比,差异无统计学意义(P>0.05).结论替普瑞酮改良四联疗法治疗Hp阳性胃溃疡患者的临床效果显著,可有效改善其胃功能与胃黏膜形态,降低机体炎症反应,提高Hp根除率,且无严重不良反应.
Objective To analyze the application effect of modified quadruple therapy of teprenone in patients with helicobacter pylori(Hp)positive gastric ulcer.Methods 72 patients with Hp positive gastric ulcer admitted to the hospital from September 2020 to July 2022 were selected as the research objects and divided into two groups according to random number table.The control group(n=36)was treated with bismuth quadruple therapy,and the observation group(n=36)was treated with modified quadruple therapy of teprenone,both groups were treated continuously for 4 weeks.The gastric function,gastric mucosal morphology,serum inflammatory factors level,clinical efficacy,Hp eradication rate and adverse reactions of the two groups were compared and analyzed.Results After treatment,the pepsinogen(PG)Ⅰand PGⅡlevels,the total effective rate and the eradication rate of Hp in the observation group were(116.91±12.46)μg/L,(5.29±1.07)μg/L,94.44%and 94.44%,respectively,which were higher than(104.53±8.27)μg/L,(4.63±1.02)μg/L,75.00%and 77.78%of the control group,the mucosal morphology score,mucosal thickness score,inflammatory cell infiltration degree score,glandular density score and C-reactive protein,interleukin(IL)-1 and IL-6 levels were(0.42±0.10)points,(0.30±0.09)points,(0.60±0.25)points,(0.38±0.07)points,(4.28±0.51)mg/L,(19.35±3.67)ng/L,(48.38±5.12)ng/L,respectively,which were lower than(0.65±0.19)points,(0.48±0.13)points,(0.78±0.29)points,(0.61±0.18)points,(6.03±0.78)mg/L,(30.17±4.28)ng/L,(71.56±9.24)ng/L of the control group,and the differences between the groups were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The modified quadruple therapy of teprenone has a significant clinical effect on HP positive gastric ulcer patients,which can effectively improve their gastric function and morphology of gastric mucosa,reduce the inflammatory reaction of the body,increase the eradication rate of Hp,and has no serious adverse reactions.
