摘要
目的探究在环状脱垂性混合痔患者中采用痔疮自动套扎术与外痔切除缝合术联合治疗对围术期相关指标、肛门功能的影响.方法选取本院2020年4月—2022年5月收治的92例环状脱垂性混合痔患者,按随机数字表法分为对照组、观察组,每组46例.对照组予以传统内扎外剥术治疗,观察组采用痔疮自动套扎术与外痔切除缝合术联合治疗.比较两组患者的围术期相关指标、肛门功能、疼痛情况及并发症发生情况.结果观察组的手术时间、创面愈合时间、住院时间分别为(15.24±1.25)min、(15.17±1.32)d、(3.23±0.58)d,均短于对照组,术中出血量为(16.56±1.03)mL,少于对照组,组间差异有统计学意义(P<0.05).术后7 d,观察组的肛管收缩压、静息压、舒张压分别为(122.05±9.23)mmHg、(53.56±3.49)mmHg、(176.25±11.65)mmHg,均高于对照组,组间差异有统计学意义(P<0.05).术后1 d、术后首次排便、术后7 d,观察组的疼痛视觉模拟评分分别为(6.60±1.08)分、(3.50±0.58)分、(1.16±0.15)分,均低于对照组,组间差异有统计学意义(P<0.05).观察组的并发症发生率为4.35%,低于对照组的19.57%,差异有统计学意义(P<0.05).结论在环状脱垂性混合痔患者中采用痔疮自动套扎术与外痔切除缝合术联合治疗,能有效改善围术期相关指标,缓解术后疼痛,降低并发症发生风险,改善肛门功能.
Objective To investigate the influence of hemorrhoid automatic ligation combined with external hemorrhoidectomy and suture on perioperative related indexes and anal function of circular prolapsed mixed hemorrhoids patients.Methods 92 circular prolapsed mixed hemorrhoids patients admitted to the hospital from April 2020 to May 2022 were selected and divided into control group and observation group by random number table method,with 46 cases in each group.The traditional internal ligation and external stripping was used in control group,the hemorrhoid automatic ligation combined with external hemorrhoidectomy and suture was used in observation group.The two groups'perioperative related indexes,anal function,pain and complications were compared.Results The observation group's operation time,wound healing time and hospital stay were(15.24±1.25)min,(15.17±1.32)d and(3.23±0.58)d,respectively,which were shorter than control group's,the intraoperative blood loss was(16.56±1.03)mL,which was less than control group's,and the differences between the groups were statistically significant(P<0.05).7 days after surgery,the observation group's anal systolic blood pressure,anal resting pressure and anal diastolic blood pressure were(122.05±9.23)mmHg,(53.56±3.49)mmHg and(176.25±11.65)mmHg,respectively,higher than control group's,and the differences between the groups were statistically significant(P<0.05).At the 1st postoperative day,the first postoperative defecation and the 7th postoperative day,the observation group's pain visual analog scale scores were(6.60±1.08)points,(3.50±0.58)points and(1.16±0.15)points,respectively,lower than control group's,and the difference between the group were statistically significant(P<0.05).The observation group's incidence of complicationswas 4.35%,lower than control group's 19.57%,the difference was statistically significant(P<0.05).Conclusion Hemorrhoid automatic ligation combined with external hemorrhoidectomy and suture in the treatment of mixed hemorrhoids with circular prolapse can effectively improve perioperative indicators,relieve postoperative pain,reduce the occurrence of related complications,and accelerate the recovery of anal function.
