摘要
目的探讨老年股骨粗隆间骨折患者采用股骨近端防旋髓内钉(PFNA)内固定联合个体化康复训练治疗的效果.方法选择本院2020年8月—2022年1月收治的老年股骨粗隆间骨折患者55例为研究对象,按随机数字表法分为对照组(n=27)及观察组(n=28).两组患者均接受PFNA内固定术治疗,术后,对照组进行常规康复训练,观察组进行个体化康复训练.比较两组患者手术前后的髋关节功能、日常生活能力,并比较两组的并发症发生率及患者对治疗的满意度.结果术后4、12周,观察组的Harris髋关节功能评分、Barthel指数评分均高于对照组,组间差异有统计学意义(P<0.05).观察组的术后并发症总发生率为3.57%,低于对照组的29.63%,差异有统计学意义(P<0.05).观察组的满意度为100.00%,高于对照组的74.07%,差异有统计学意义(P<0.05).结论老年股骨粗隆间骨折患者采用PFNA内固定联合个体化康复训练治疗,可促进关节功能恢复,提高生活能力,患者对治疗的满意度高,值得临床推广使用.
Objective To investigate the effect of proximal femoral nail anti-rotation(PFNA)internal fixation combined with individualized rehabilitation training in elderly patients with intertrochanteric fracture.Methods A total of 55 elderly patients with intertrochanteric fracture admitted to our hospital from August 2020 to January 2022 were selected as the research objects and were divided into a control group(n=27)and an observation group(n=28)according to the random number table method.Both groups were treated with PFNA internal fixation.After operation,the control group received routine rehabilitation training,while the observation group received individualized rehabilitation training.The hip function and daily living ability of the two groups were compared before and after operation,and the incidence of complications and patients'satisfaction with treatment were compared between the two groups.Results At 4 and 12 weeks after operation,the Harris hip function score and Barthel index score of the observation group were higher than those of the control group,and the differences between the groups were statistically significant(P<0.05).The total incidence of postoperative complications in the observation group was 3.57%,which was lower than 29.63%in the control group,and the difference was statistically significant(P<0.05).The satisfaction of the observation group was 100.00%,which was higher than 74.07%of the control group,and the difference was statistically significant(P<0.05).Conclusion PFNA internal fixation combined with individualized rehabilitation training in elderly patients with intertrochanteric fracture can promote the recovery of joint function and improve the ability of life.Patients have high satisfaction with the treatment,which is worthy of clinical application.
