摘要
目的探讨早期康复训练结合中医针灸在胫骨平台骨折术后功能障碍患者康复中的应用效果.方法选取2015年7月—2020年12月中国人民解放军94600部队医院收治的160例胫骨平台骨折术后功能障碍患者为研究对象,随机将其分为两组,对照组80例进行早期康复训练,观察组80例在此基础上采用中医针灸治疗.对比两组的临床疗效、膝关节相关指标、疼痛程度、骨折愈合时间、生活质量及舒适度.结果观察组的治疗优良率为96.25%,明显高于对照组的82.50%,组间差异有统计学意义(P<0.05).观察组治疗后1周、1、3个月的美国特种外科医院(HSS)膝关节评分、膝关节活动度(ROM)均明显高于对照组,组间差异有统计学意义(P<0.05).观察组治疗后的疼痛视觉模拟评分法(VAS)评分明显低于对照组,骨折愈合时间明显短于对照组,组间差异有统计学意义(P<0.05).观察组治疗后的健康调查简表(SF-36)评分明显高于对照组,舒适度评分明显低于对照组,组间差异有统计学意义(P<0.05).结论早期康复训练结合中医针灸在胫骨平台骨折术后功能障碍患者康复中的应用效果显著,能有效提高骨折愈合效果,缩短骨折愈合时间,改善膝关节功能及膝关节活动度,并能减轻疼痛,提高舒适度,提升生活质量,具有积极的临床意义.
Objective To explore the effect of early rehabilitation training combined with TCM acupuncture and moxibustion in rehabilitation of patients with functional dysfunction after tibial plateau fracture.Methods A total of 160 patients with postoperative dysfunction of tibial plateau fracture treated in 94600 Army Hospital of the People's Liberation Army from July 2015 to December 2020 were selected as the research objects,and they were randomly divided into two groups.80 cases in the control group received early rehabilitation training,and 80 cases in the observation group received TCM acupuncture treatment on this basis.The clinical efficacy,knee related indexes,pain degree,fracture healing time,quality of life and comfort were compared between the two groups.Results The excellent and good rate of the treatment in the observation group was 96.25%,which was significantly higher than 82.50%in the control group,and the difference was statistically significant(P<0.05).The HSS knee score and ROM of the observation group were significantly higher than those of the control group at 1 week,1 and 3 months after treatment,and the differences between the groups were statistically significant(P<0.05).The visual analogue scale(VAS)score of pain after treatment in the observation group was significantly lower than that in the control group,and the fracture healing time was significantly shorter than that in the control group,the differences between the groups were statistically significant(P<0.05).After treatment,the MOS item short from health survey(SF-36)score of the observation group was significantly higher than that of the control group,and the comfort score was significantly lower than that of the control group,the differences between the groups were statistically significant(P<0.05).Conclusion Early rehabilitation training combined with TCM acupuncture in postoperative dysfunction in patients with tibial plateau fracture healing effect is remarkable,the application can effectively improve the effect of fracture healing,shorten fracture healing time,improve the function and activity of knee joint,and can relieve pain,improve comfort and the quality of life,which has positive clinical significance.
